接传红从虚实两端立论治疗糖尿病性视网膜病变经验药对

摘　　要：糖尿病性视网膜病变(DR)是糖尿病(DM)患者眼部的微血管病变,主要表现为视网膜微动脉瘤、出血、渗出、增殖等。接传红治疗DR以虚实两端立论,认为病机总属本虚标实,痰瘀为患,虚实夹杂,治疗上应辨明虚实轻重,合理用药,常在密蒙花方、生蒲黄汤、知柏地黄汤、补阳还五汤等基础方上加减治疗,并配伍应用特色的经验药对。常用特色药对有祛瘀生新之“大黄-丹参”、活血通络之“石菖蒲-路路通-鸡血藤”、祛湿化痰之“陈皮-法半夏-车前子”、解郁散结之“香附-郁金”;养血柔肝之“柴胡-白芍”、滋补肾精之“黄精-熟地黄”、培土制水之“茯苓-薏苡仁-益母草”、养阴生津之“麦冬-五味子-石斛”、温补阳气之“益智仁-肉桂”,临床疗效较好。Diabetes retinopathy(DR)is a microangiopathy in the eyes of diabetes mellitus(DM)patients,mainly manifested as retinal microaneurysms,hemorrhage,exudation,proliferation,etc.JIE Chuanhong treats DR based on both deficiency and excess.She believes that generally,the pathogenesis of DR is root deficiency and tip excess,phlegm and blood stasis,and deficiencyexcess complex.In the treatment,it is necessary to distinguish whether deficiency or excess is in dominance,and accordingly use drugs reasonably.In general,modified prescriptions of Mimenghua Formula,Shengpuhuang Decoction,Zhibai Dihuang Decoction,Buyang Huanwu Decoction are used,along with characteristic empirical medicine pairs.Commonly used characteristic drug pairs include"rhubarb-salvia miltiorrhiza"for removing blood stasis and generating new blood,"acorus calamus-lulutong-caulis spatholobi"for activating blood circulation and unblocking collaterals,"tangerine peel-French pinellia-plantain seed"for removing dampness and resolving phlegm,and"Xiangfu-Yujin"for resolving depression and dispersing stagnation;"Bupleurum chinense-white peony"for nourishing blood and softening liver,"yellow essence-cooked ground"for nourishing kidney essence,"poria cocos-coix seed-motherwort"for cultivating earth to control water,"ophiopogon japonicus-schisandra chinensis-dendrobium"for tonifying Yin and engendering fluid,and"yizhiren-cinnamon"for warming and tonifying Yang.
