基于医案分析岭南名老中医治疗慢性肝病的用药规律及特色

摘　　要：目的:本研究立足于岭南中医的继承,通过分析医案数据以总结岭南名老中医治疗慢性肝病(慢性肝炎及肝硬化)的用药规律及特色。方法:通过检索国家知识基础设施数据库(CNKI)、广州中医药大学馆藏书籍,收集岭南名老中医治疗慢性肝病的医案,利用SPSS modeler 15.0、Cytoscape 3.7.1等软件进行医案处方的数据挖掘。结果:最终筛选出相关医家32名、医案处方157个;医案处方涉及中药230种,白术、白芍、甘草、茯苓、柴胡、丹参6味中药为核心组方用药;系统聚类分析显使用的药物以扶正、理气、活血、清热、利湿、攻坚消积中药等为主。结论:岭南名老中医治疗慢性肝病采用疏肝健脾、柔肝活血为治疗大法,常配合清热解毒、利湿退黄、软坚消积之品,且善用岭南道地药材,具有岭南地域特色。Objective:This study is based on the inheritance of traditional Chinese medicine(TCM)in Lingnan(south of the Five Ridges).By analyzing the medical records,this study summarized the medication rules and characteristics for chronic liver diseases(chronic hepatitis and liver cirrhosis)treated by famous and veteran TCM physicians in Lingnan.Methods:By searching the China national knowledge infrastructure(CNKI)database and the books in Guangzhou University of Traditional Chinese Medicine,the medical records written by famous and veteran TCM physicians in Lingnan in treating chronic liver diseases were collected,and SPSS modeler 15.0,Cytoscape 3.7.1,and other software were used for data mining of prescriptions in medical records.Results:Thirty two physicians and 157 prescriptions in medical records were found.The prescriptions involved 230 kinds of TCM,with Macrocephalae Rhizoma,Paeoniae Radix Alba,Radix Rhizoma Glycyrrhizae,Poria,Radix Bupleuri,and Salviae Miltiorrhizae as the core drugs.The result of system cluster analysis showed that the drugs used had the effects of strengthening vital qi,regulating qi-flowing,promoting blood circulation,clearing heat,promoting diuresis,softening hardness,and resolving accumulation.Conclusion:In the treatment of chronic liver diseases,famous and veteran TCM physicians in Lingnan adopted the methods of soothing the liver,invigorating the spleen,softening the liver,and promoting blood circulation,which were often combined with the products for clearing heat,detoxifying,dispelling dampness,eliminating icterus,softening hardness,and resolving accumulation.They also made good use of Chinese herbs in Lingnan,which had regional characteristics.
