清肠合剂灌肠联合中药穴位贴敷对腹腔镜阑尾切除术患者术后康复的影响

摘　　要：目的:探讨清肠合剂灌肠联合中药穴位贴敷对腹腔镜阑尾切除术患者术后胃肠功能恢复、疼痛情况及胃动素、白细胞介素(IL)-6水平的影响。方法:将126例腹腔镜阑尾切除术后患者按随机数字表法分为对照组和观察组各63例,对照组给予西医基础治疗与肥皂水灌肠,观察组给予西医基础治疗,同时应用清肠合剂灌肠联合中药穴位贴敷,比较两组患者术后首次肠鸣音恢复时间、首次肛门排气时间、首次排便时间、疼痛视觉模拟评分法(VAS)评分,并对两组患者治疗前后血浆胃动素、IL-6水平进行比较。结果:观察组术后首次肠鸣音恢复时间、首次肛门排气时间、首次排便时间及VAS评分均优于对照组,差异有统计学意义(P<0.05)。两组患者治疗后血浆胃动素水平均较治疗前升高,IL-6水平均较治疗前降低,差异均有统计学意义(P<0.05);且治疗后观察组胃动素水平高于对照组,IL-6水平低于对照组,差异均有统计学意义(P<0.05)。结论:应用清肠合剂灌肠联合穴位贴敷能够加快腹腔镜阑尾切除术患者术后胃肠功能恢复,减轻炎症反应,促进患者的术后康复。Objective:To investigate the effects of Qingchang Mixture(清肠合剂)enema combined with Chinese medicine acupoint application on gastrointestinal function recovery,pain,motilin and interleukin-6(IL-6)levels in patients after laparoscopic appendectomy.Methods:A total of 126 patients after laparoscopic appendectomy were divided into the control group and the observation group according to random number table method,with 63 cases in each group.The control group received basic western medicine treatment and soapy water enema,while the observation group received basic western medicine treatment,and Qingchang Mixture enema combined with Chinese medicine acupoint application.The first bowel sound recovery time,first anal exhaust time,first defecation time and pain visual analog scale(VAS)score were compared between the two groups after treatment,and the plasma levels of motilin and IL-6 were compared between the two groups before and after treatment.Results:The postoperative first bowel sound recovery time,first anal exhaust time,first defecation time and VAS score in the observation group were better than those in the control group,and the differences were statistically significant(P<0.05).After treatment,plasma motilin level in both groups was higher than that before treatment,and IL-6 level was lower than that before treatment,and the differences were statistically significant(P<0.05).After treatment,the motilin level in the observation group was higher than that in the control group,and IL-6 level was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusions:The application of Qingchang Mixture enema combined with acupoint application can accelerate the recovery of gastrointestinal function,reduce inflammation and promote postoperative rehabilitation of patients after laparoscopic appendectomy.
