基于CBCT对汉族成人右下颌第一恒磨牙髓室参数与年龄关系的研究

摘  要:目的应用CBCT研究汉族成人右下颌第一恒磨牙髓室参数与年龄的关系;构建汉族成人右下颌第一恒磨牙髓室参数年龄推断模型。方法收集川北医学院附属医院口腔科汉族成人牙齿CBCT影像资料共1654例(男、女各827例),分5组(18~27岁、28~37岁、38~47岁、48~57岁、≥58岁)。运用Mimics软件测量牙齿中轴线上髓室参数(髓室的高度、髓室顶牙本质厚度、髓室底牙本质厚度、髓室宽度、髓室颊侧壁牙本质厚度、髓室舌侧壁牙本质厚度)并进行统计分析,1354例样本构建髓室参数的年龄推断模型,300份样本进行模型验证。结果髓室高度和髓室颊侧壁牙本质厚度在不同年龄段中均存在显著差异;所有参数均与年龄有关,髓室高度、髓室宽度与年龄呈负相关,以髓室高度相关性最高(r=-0.58),髓室顶牙本质厚度、髓室底牙本质厚度、髓室颊侧壁牙本质厚度、髓室舌侧壁牙本质厚度与年龄呈正相关,以髓室顶牙本质厚度相关性最高(r=0.56);逐步回归显示年龄=53.564-6.80X_(1)-8.904X_(2)+5.984X_(3)+1.676X_(4)模型较优(R^(2)=0.625),平均绝对误差为5.9岁。结论汉族成人右下颌第一恒磨牙髓室相关参数与年龄具有较好的相关性,能够为法医学年龄推断提供新的方法。Objective To study the relationship between the age and the pulp chamber parameters of the right mandibular first permanent molar in Han adults by CBCT,and to construct an age inference model based on the pulp chamber parameters of the right mandibular first permanent molar in Han adults.Methods The CBCT imaging data of 1654 Han adults(827 males and 827 females)from Department of Stomatology,Affiliated Hospital of North Sichuan Medical College,were collected and divided into 5 groups(18~27 years old,28~37 years old,38~47 years old,48~57 years old,≥58 years old).The Mimics software was used to measure the pulp chamber parameters(the height of the pulp chamber,the dentin thickness of the pulp chamber roof,the dentin thickness of the pulp chamber floor,the width of the pulp chamber,the dentin thickness of the buccal wall of the pulp chamber,and the dentin thickness of the lingual wall of the pulp chamber)and statistical analysis were performed.The data of 1354 samples were used to build an inference model with the pulp chamber parameters and the age,and the data of the other 300 samples were used for model validation.Results There were significant differences in the pulp chamber height and the pulp chamber buccal wall dentin thickness in all age groups.All parameters were related to age.The pulp chamber height and width were negatively correlated with age,and the pulp chamber height had the most significant correlation(r=-0.58).The dentin thickness of the roof,floor,buccal wall and lingual wall of the pulp chamber were positively correlated with age,and the dentin thickness of the pulp chamber roof had the most significant correlation(r=0.56).Stepwise regression showed that the model age=53.564-6.80x1-8.904x2+5.984x3+1.676x4 was the best(R^(2)=0.625),and the mean absolute error was 5.9 years.Conclusion The pulp chamber parameters of the right mandibular first permanent molar in Han adults have a good correlation with age,which can provide a new method for forensic age inference.
