摘　　要：目的探讨稳定型帽状移植物在鼻尖整形中的应用效果。方法回顾性分析自2020年2月至2022年6月,重庆市两江新区人民医院医美抗衰容中心和贵阳市第二人民医院整形美容科采用稳定型帽状移植物处理50例鼻尖形态欠佳患者。首先将获取的自体肋软骨在术中构建牢固的鼻尖支架结构,再将帽状移植物与支架以“榫卯”的方式进行固定,形成稳定型“帽状”移植物。通过观察术后是否出现鼻尖下旋、鼻尖表现点下移等并发症,以及术后观察患者鼻尖形态、鼻尖高度、鼻尖下小叶的饱满度等,对稳定型“帽状”移植物在鼻尖整形中的应用效果进行探讨。结果50例患者获随访12~24个月,平均18个月,1例鼻尖下小叶欠饱满,鼻尖表现点不对称;其余49例鼻尖形态、鼻尖高度、鼻尖下小叶饱满度均得到有效改善。术后无鼻尖下旋、鼻尖表现点下移等并发症发生,效果满意。结论在鼻尖整形手术中应用稳定型“帽状”移植物可有效地防止术后鼻尖下旋、鼻尖表现点下移等并发症的发生,并且能有效改善患者鼻尖形态、鼻尖高度及鼻尖下小叶饱满度。Objective To investigate the effect of stable cap grafts in rhinoplasty.Methods From February 2020 to June 2022,50 patients with suboptimal nasal tip morphology treated with a stable cap graft were collected.The excised autologous rib cartilage was firstly used to construct a strong nasal tip scaffold structure intraoperatively,and then the cap grafts were fixed with the scaffold in a"mortise and tenon joint"manner to form a stable cap graft.We also observed whether there were complications such as downward rotation of nasal tip and downward movement of nasal tip presentation point after operation,and observed the shape of nasal tip,the height of nasal tip and the plumpness of inferior lobular of nasal tip of patients after operation,so as to discuss the application effect of stable"cap-shaped"graft in nasal tip plasty.Results After 12-24 months of postoperative follow-up,with the average follow-up time of 18 months,in 50 patients,the lower nasal tip leaflet was insufficient,which was effective,the inferior tip leaflet was not full in 1 case,and the nose tip was asymmetric,the nasal tip morphology,nasal tip height and lower nasal tip fullness of the remaining 49 patients were effectively improved,which was significantly effective.None of the 50 patients had postoperative complications such as nasal tip downrotation and nasal tip presentation downpoint shift.Conclusion The application of stable cap grafts in rhinoplasty can effectively prevent postoperative complications such as nasal tip downward rotation and nasal tip presentation point downward shift,and can effectively improve the nasal tip morphology,nasal tip height and subtip leaflet full of patients.
