体位矫正与头型矫正器治疗姿势性斜头畸形的临床效果比较

摘　　要：目的比较体位矫正与头型矫正器在婴儿姿势性斜头畸形治疗中的临床效果。方法对2014年9月至2022年2月在和睦家京北妇儿医院诊治的60例3~7月龄姿势性斜头畸形患儿进行研究,按照不同的治疗方式分为体位矫正治疗组(n=30)和头型矫正器治疗组(n=30)。对两组患儿的头颅不对称指数(CVAI)进行比较。结果两组治疗后CVAI均低于治疗前,CRO治疗组治疗后CVAI值[(3.8±1.8)%]低于体位矫正治疗组[(4.7±1.8)%](P<0.05)。CRO治疗组治疗前后的CVAI差值高于体位矫正治疗组[2.5(1.9,3.4)%vs 1.3(0.2,1.7)%,Z=4.77,P<0.001]。结论对于3~7月龄姿势性斜头畸形患儿,体位矫正治疗及头型矫正器治疗均可以获得显著的效果。头型矫正器可以使患儿获得更好的预后,治疗后头颅的对称性优于体位矫正。Objective To compare the clinical effect of repositioning and cranial remolding orthoses(CRO)in the treatment of positional plagiocephaly.Methods The 60 children with positional plagiocephaly aged 3 to 7 months old treated in Beijing United Family Women’s&Children’s Hospital from September 2014 to February 2022 were selected and divided into repositioning treatment group(n=30)and CRO treatment group(n=30)according to different treatment methods.The cranial vault asymmetry index(CVAI)was compared between the two groups.Results The CVAI after treatment in both groups was lower than that before treatment,and the CVAI value in the CRO treatment group([3.8±1.8]%)after treatment was lower than that in the repositioning treatment group([4.7±1.8]%)(P<0.05).The difference in CVAI before and after treatment in the CRO treatment group was higher than that in the repositioning treatment group(2.5[1.9,3.4]%vs 1.3[0.2,1.7]%,Z=4.77,P<0.001).Conclusion Both repositioning and CRO treatment for children with positional plagiocephaly aged 3 to 7 months old can achieve significant results.The CRO treatment can achieve a better prognosis,and the symmetry of t he head is better than that of repositioning.
