摘　　要：目的评估心脏磁共振(CMR)定量参数在致心律失常右室心肌病(ARVC)患者中的诊断价值。方法回顾性收集郑州大学人民医院行3T心脏磁共振检查的ARVC患者43例(ARVC组)及30例健康志愿者(对照组)。测量两组左室舒张末期容积指数、右室舒张末期容积指数,左室射血分数、右室射血分数、左室质量指数和初始T1mapping值,根据ARVC组患者心室受累情况分为孤立右心室受累(A组)、双心室受累(B组)、孤立左心室受累(C组),分别比较ARVC组与正常对照组及ARVC各亚组间相关参数的差异。结果ARVC组左室舒张末期容积指数、右室舒张末期容积指数、左室质量指数和初始T1mapping值均高于正常对照组[两组分别为(95±21)mL/m^(2)和(73±14)mL/m^(2)、(107±30)mL/m^(2)和(81±9)mL/m^(2)、(76±18)g/m^(2)和(56±11)g/m^(2)、(1306±79)ms和(1182±49)ms],ARVC组左室射血分数、右室射血分数均低于正常对照组[两组分别为(46±10)%和(58±5)%、(37±10)和(54±7)%],差异均有统计学意义(P值均<0.05);A组、B组、C组和正常对照组在左室舒张末期容积指数、右室舒张末期容积指数、射血分数以及T1mapping值等定量参数间存在差异,差异均有统计学意义(P值均<0.05)。A组和B组、B组和C组在室壁运动异常、心肌延迟强化、脂肪浸润等定性指标间差异不显著(P值均>0.05)结论心脏磁共振(CMR)舒张末期容积指数、左室质量指数、T1mappin值等参数可定量评估ARVC的心肌组织学特征,有重要的诊断价值。Objective To evaluate the diagnostic value of cardiac magnetic resonance(CMR)quantitative parameters in patients with arrhythmic right ventricular cardiomyopathy(ARVC).Methods Totally 43 patients with ARVC underwent 3T cardiac magnetic resonance examination(ARVC group)and 30 healthy volunteers(control group)were retrospectively collected.Left ventricular end-diastolic volume index,right ventricular end-diastolic volume index,left ventricular ejection fraction,right ventricular ejection fraction,left ventricular mass index and initial Tlmapping values were measured in the two groups.According to the ventricular involvement of ARVC group,patients were divided into isolated right ventricular involvement(group A),double ventricular involvement(group B)and isolated left ventricular involvement(group C).The differences of related parameters between ARVC group and normal control group and ARVC subgroups were compared.Results Left ventricular end-diastolic volume index,right ventricular end-diastolic volume index,left ventricular mass index and initial TImapping values in THE ARVC group were higher than those of the normal control group[two groups were(95±21)mL/m^(2)and(73±14)mL/m^(2),(107±30)mL/m^(2)and(81±9)mL/m^(2),(76±18)g/m^(2)and(56±11)g/m^(2),(1306±79)ms and(1182±49)ms,respectively],the left ventricular ejection fraction and right ventricular ejection fraction in ARVC group were lower than those of normal control group[(46±10)%and(58±5)%,(37±10)and(54±7)%,respectively],and the differences were statistically significant(P<0.05).There were significant differences in left ventricular end-diastolic volume index,right ventricular end-diastolic volume index,ejection fraction and Tlmapping value among group A,B,C and normal con trol group(all P<0.05).There were no significant differences between group A and group B,group B and group C in the qualitative indexes such as ventricular wall motion abnormalities,myocardial delayed reinforcement,fatty infiltration(P>0.05).Conclusion Cardiac magnetic resonance(CMR)en
