摘 要:目的调查空军飞行人员颈腰痛的患病情况及功能障碍程度,为飞行人员颈腰痛防治提供依据。方法采用自制调查量表,结合颈椎功能障碍指数(neck disability index,NDI)和腰椎功能障碍指数(Oswestry disability index,ODI)问卷量表统计分析飞行人员的颈腰痛患病情况。比较不同岗位、机种、年龄、飞行时间飞行人员的颈腰痛患病情况;分析飞行人员发生颈腰痛的危险因素。结果①战/技勤人员腰痛的患病率高于飞行员,差异有统计学意义(χ^(2)=4.88,P=0.027)。运输机飞行人员颈痛患病率高于歼击机飞行员,差异有统计学意义(P=0.001)。运输机和轰炸机飞行人员NDI高于歼击机飞行员(P值均=0.001),运输机飞行人员ODI高于歼击机飞行员和轰炸机飞行人员,差异有统计学意义(P=0.001、0.024)。②不同飞行时间歼击机飞行员颈腰痛患病率差异有统计学意义(χ^(2)=7.96,P=0.022);不同飞行时间运输机/轰炸机飞行人员NDI差异有统计学意义(H=9.53,P=0.023),飞行时间>2000h的运输机/轰炸机飞行人员颈部NDI高于飞行时间<500h的运输机/轰炸机飞行人员,差异有统计学意义(P=0.041)。③不同年龄组运输机/轰炸机飞行人员NDI和腰痛患病率组间差异有统计学意义(H=13.10,χ^(2)=7.81,P=0.001、0.020)>40岁组NDI高于其他年龄组,差异有统计学意义(P=0.002、0.029);>40岁组腰痛的患病率高于31~40岁组,差异有统计学意义(P=0.013)。④飞行机种是飞行人员发生颈腰痛的危险因素(P=0.009),OR=3.06,P<0.001。结论战/技勤人员腰痛患病率比飞行员高。运输机飞行人员颈腰痛患病率高于其他机种。Objective To provide references for the prevention and treatment of neck and low back pain in flying personnel by investigating the prevalence status of neck and low back pain and the degree of dysfunction.Methods A self-made questionnaire combined with neck disability index(NDI)and Oswestry disability index(ODI)were used to analyze the detection rate of neck and low back pain in flying personnel.The prevalence status of neck and low back pain was compared among different post,types of aircraft,age and flying hours.The risk factors of neck and low back pain were analyzed in flying personnel.Results①The prevalence rate of low back pain in combat/technical personnel were higher than that in pilots,and the difference was statistically significant(χ^(2)=4.88,P=0.027).The prevalence rate of neck pain in transporter flying personnel were higher than that in fighter pilots,and the difference was statistically significant(P=0.001).NDI of transporter and bomber flying personnel were higher than that in fighter pilots(all P=0.001).ODI of transporter flying personnel were higher than that in fighter pilots and bomber flying personnel,and the differences were statistically significant(P=0.001,0.024).②There was significant difference in the prevalence rate of neck and low back pain among fighter pilots with different flying hours(χ^(2)=7.96,P=0.022);there was significant difference in the NDI of transporter/bomber flying personnel with different flying hours(H=9.53,P=0.023).NDI of transporter/bomber flying personnel with flying hours>2000 h was higher than that of transporter/bomber flying personnel with flying hours<500 h,and the difference was statistically significant(P=0.041).③There were significant differences in the NDI and the prevalence rate of low back pain among transporter/bomber flying personnel with different age groups(H=13.10,χ^(2)=7.81,P=0.001,0.020).NDI of the transporter/bomber flying personnel older than 40 years was higher than that in other age groups,and the differences were statistically signif
