黄芪补肾活血汤联合西医疗法治疗早期糖尿病肾病的疗效及对患者血糖、肾功能水平的影响

摘　　要：目的探究黄芪补肾活血汤联合西医疗法治疗早期糖尿病肾病(diabetic kidney disease,DKD)的疗效,以及对DKD患者血糖、肾功能水平的影响。方法选取2020年4月—2022年4月在齐齐哈尔市第一医院确诊的80例DKD患者为研究对象,随机分为西医治疗组(40例,口服二甲双胍和氯沙坦)和中西医联合治疗组(40例,口服二甲双胍、氯沙坦和黄芪补肾活血汤)。对比两组治疗效果。结果治疗8周后,西医治疗组中医症候积分为(16.89±3.29)分,显著高于中西医联合治疗组的(12.45±2.11)分,差异有统计学意义(t=7.185,P<0.05)。西医治疗组临床总有效率为67.5%,显著低于中西医联合治疗组90.0%,差异有统计学意义(P<0.05)。治疗前,两组患者的FBG、2 hPG、HbA1c、BUN、Scr、24 hUP、UAER水平比较,差异无统计学意义(P>0.05);治疗8周后,中西医联合治疗组的各指标均显著低于西医治疗组,差异有统计学意义(P<0.05)。结论黄芪补肾活血汤联合西医疗法治疗早期糖尿病肾病效果显著,有效降低血糖水平,改善肾功能的作用。Objective To explore the curative effect of Huangqi Bushen Huoxue Decoction combined with western medicine on early diabetic kidney disease,and the effect on blood glucose and renal function of DKD patients.Methods Eighty patients with DKD diagnosed in the First Hospital of Qiqihar from April 2020 to April 2022 were selected for the study.The patients were randomly divided into Western medicine treatment group(40 cases,oral Metformin and Losartan)and combined Western and Chinese medicine treatment group(40 cases,oral Metformin,losartan and Huangqi Bushen Huoxue Decoction).The treatment effects of the two groups were compared.Results After 8 weeks of treatment,the TCM symptom score in the Western medicine treatment group was(16.89±3.29)points,which was significantly higher than that in the combined Western and Chinese medicine treatment group(12.45±2.11)points,the difference was statistically significant(t=7.185,P<0.05).The total clinical effective rate of the Western medicine treatment group was 67.5%,which was significantly lower than that of the combined Western and Chinese medicine treatment group of 90.0%,the difference was statistically significant(P<0.05).Before treatment,the levels of FBG,2 hPG,HbA1c,BUN,Scr,24 hUP and UAER in the two groups were compared,the difference was not statistically significant(P>0.05).After 8 weeks of treatment,all indexes in the combined Chinese and Western medicine treatment group were significantly lower than those in the Western medicine treatment group,the difference was statistically significant(P<0.05).Conclusion Huangqi Bushen Huoxue Decoction combined with western medicine therapy is effective in treating early diabetic nephropathy,which can effectively reduce blood glucose level and improve renal function.
