益气温阳祛湿通络中药治疗早中期慢性肾功能衰竭临床疗效观察

摘　　要：目的:观察益气温阳祛湿通络中药治疗早中期慢性肾功能衰竭(CRF)的治疗效果。方法:选取2014年1月至2016年12月就诊于河北省中医院、河北医科大学第三医院、河北省沧州中西医结合医院的早中期CRF患者147例。随机分为对照组(74例)、观察组(73例),两组患者均接受基础治疗,对照组加用尿毒清颗粒,观察组辨证加用益气温阳祛湿通络中药,共观察6个月。治疗前后分别观察两组患者临床症状改善情况、临床疗效及血肌酐(Scr)、尿素氮(BUN)、肾小球滤过率(GFR)、24 h尿蛋白定量(24 h UTP)、血红蛋白(Hgb)、血浆白蛋白(ALB)等变化。结果:治疗后,观察组有效率[84.06%(58/69)]显著高于对照组[68.57%(48/70)](P<0.05)。与对照组比较,观察组能显著改善CRF患者临床症状(倦怠乏力、腰膝酸软、食少纳呆、恶心呕吐、大便干结)及Scr、BUN、GFR、Hgb、ALB(P<0.05)。结论:益气温阳祛湿通络中药可以保护早中期CRF患者肾功能、改善临床症状、延缓病程进展、提高生活质量,临床应用安全有效。Objective:To observe the therapeutic effect of Yiqi Wenyang Qushi Tongluo herbs in the treatment of patients with early and middle stage chronic renal failure(CRF).Methods:A total of 147 early and middle stages CRF patients treated in Hebei Traditional Chinese Medicine Hospital,The Third Hospital of Hebei Medical University and Cangzhou Hospital of Integrated TCM-WM·Hebei from January 2014 to December 2016 were randomly divided into control group(74 cases)and observation group(73 cases).Both groups were given with basic western medicine treatment,the control group was treated with Niaoduqing Granule,and the observation group was treated withYiqi Wenyang Qushi Tongluo herbs for 6 months.The improvement of clinical symptoms,clinical effective,24-hour urinary protein quantity(24 h UTP),serum(Scr),urea nitrogen(BUN),glomerular filtrate rate(GFR),hemoglobin(Hgb)and plasma albumin(ALB)were detected before and after the treatment.Results:The effective rate was 84.06%(58/69)in the observation group,it was higher than that in the control group[68.57%(48/70)](P<0.05).Compared with the control group,the observation group could significantly improve the clinical symptoms(languor,lumbar debility,food less poor appetite,nausea and vomiting,dry stool),Scr,BUN,GFR,Hgb,ALB(P<0.05).Conclusion:Yiqi Wenyang Qushi Tongluo herbs can protect renal function,improve clinical symptoms,delay the course of disease,improve the quality of life,and be safe and effective in clinical application of CRF patients in the early and middle stages.
