脾肾阳虚型CFS患者不同施灸时长长蛇灸透热感规律研究

摘　　要：目的:以脾肾阳虚性慢型疲劳综合征(chroninc fatigue syndrome,CFS)患者为载体,研究透热感在不同施灸时长长蛇灸治疗过程中的规律,以期丰富灸感-灸时的量学关系,为长蛇灸临床方案优化提供科学依据。方法:将60例CFS患者1:1随机分为60分钟组(A组)和30分钟组(B组)接受长蛇灸治疗,每周3次(隔日一次),连续治疗4周,共12次,并于每次治疗后采用VAS评价透热感,及医用红外额温计测膻中穴、神阙穴治疗前后的体表温度。结果:分组及治疗次数对透热感及穴位温差值无交互作用,分组及治疗次数对穴位温差值的影响均不显著,但治疗次数对透热感强度的主效应具有统计学意义。A组随着治疗次数增加,先于B组出现最强透热感,并获得饱和灸量;B组随着治疗次数增加透热感持续上升,但在12次治疗结束时仍未获得透热感峰值。结论:基于透热感变化规律,对于以长蛇灸为代表的大面积灸法,疗程是影响饱和灸量获得的关键因素,而非单次治疗时长。Objective To study the law of heat penetration of long-snake-like moxibustion with different treatment duration in patients with CFS of spleen and kidney Yang deficiency,in order to enrich the quantitative relationship between moxibustion feeling and treatment duration or course,and provide scientific basis for the optimization of clinical scheme of long-snake-like moxibustion.Methods Patients with CFS were randomly divided into 60 minute group(group A)and 30 minute group(group B).They were treated with long-snake-like moxibustion three times a week(once every other day)for 4 weeks,with a total of 12 times.After each treatment,VAS was used to evaluate the heat penetration sensation,and medical infrared forehead thermometer was used to measure the body surface temperature of Tanzhong point(RN17)and Shenque point(RN8)before and after treatment.Results There was no interaction between group and treatment times on heat penetration sensation and acupoint temperature difference.Grouping and treatment times had no significant effect on acupoints temperature difference,but the main effect of treatment times on heat penetration sensation was statistically significant.With the increase of treatment times,group A had the highest heat penetration sensation earlier than group B,and obtained the saturated moxibustion dosage;In group B,the heat penetration sensation continued to rise with the increase of treatment times,but the peak value of diathermy was not obtained at the end of 12 treatments.Conclusion For the large-area moxibustion represented by long-snake-like moxibustion,the course of treatment is the key factor affecting the dosage of moxibustion,rather than the treatment duration.
