摘 要:目的探讨改良PICC固定套在神经外科患者中线导管(MC)固定中的应用效果。方法选取医院神经外科2021年1—6月行MC置入的50例患者作为对照组,采用常规方法进行导管固定;2021年7—12月行MC置入的54例患者作为观察组,在对照组基础上应用改良PICC固定套进行MC导管外固定,两组均按照标准MC导管置入及维护流程。比较两组患者带管期间MC相关并发症发生率、提前换药发生率、置管部位舒适度评分及满意度。结果观察组患者MC相关并发症(渗血渗液、移位、Ⅱ级静脉炎、导管堵塞)发生率低于对照组,差异有统计学意义(P<0.05);观察组患者提前换药发生率低于对照组,差异有统计学意义(P<0.05);观察组患者置管处舒适度及满意度均优于对照组,差异有统计学意义(P<0.05)。结论应用改良PICC固定套可降低神经外科患者MC相关并发症、意外拔管发生率及提前换药发生率,并改善患者置管部位舒适度,有效提高患者满意度。Objective To investigate the effect of the modified PICC fixation sleeve in the fixation of midline catheter(MC)in neurosurgical patients.Methods 50 patients who underwent MC placement from January to June 2021 in the neurosurgery department of the hospital were selected as the control group,and the catheter fixation was performed by conventional methods.Both groups followed the standard MC catheter insertion and maintenance procedure.The incidence of MC-related complications,the incidence of early dressing changes,the comfort level of the placement site and the satisfaction level of the patients in the two groups were compared.Results The incidence of MC-related complications(blood and fluid leakage,displacement,grade II phlebitis and catheter occlusion)was lower in the observation group than in the control group,and the difference was statistically significant(P<0.05);the incidence of early dressing changes was lower in the observation group than in the control group,the difference was statistically significant(P<0.05);the comfort and satisfaction level at the placement site were better in the observation group than in the control group,the difference was statistically significant(P<0.05).The difference was statistically significant(P<0.05).Conclusion The use of the modified PICC fixation set could reduce the incidence of MC-related complications,unplanned extubation and early medication changes in neurosurgical patients,and improve the comfort of patients at the placement site and effectively increase patient satisfaction.
