摘 要:目的探讨二维斑点追踪分层应变技术评估系统性红斑狼疮(SLE)患者左室各层心肌收缩功能的价值.方法选取50例SLE患者为SLE组,分析二维斑点追踪分层应变技术对SLE患者的诊断价值,另选取50名健康体检者作为对照组,比较两组超声心动图检查结果、整体应变指标.此外,将SLE组患者按照病程分组,其中病程<3 a患者为SLEⅠ组(n=22),病程≥3 a的患者为SLEⅡ组(n=28),对比两组超声心动图检查结果、整体应变指标.结果SLE组与对照组比较,常规超声心动图指标LVEF、LVEDd、PWT、LVFS、IVST比较差异无统计学意义(P>0.05),SLE组整体应变指标GLS、GCS、LVTW比较差异具有统计学意义(P<0.05).SLEⅠ组与SLEⅡ组比较,常规超声心动图指标中LVEF、LVFS差异具有统计学意义(P<0.05);其余指标差异无统计学意义(P>0.05).SLEⅠ组与SLEⅡ组比较,GLS、GCS、LVTW指标差异均具有统计学意义(P<0.05).结论SLE患者存在不同程度的左心室功能受损情况,通过常规超声难以检出,通过二维斑点追踪分层应变技术能够早期检出左心室受损情况,为患者及早进行干预赢得时机,有助于患者获得更为理想的结局.Objective To explore the value of two-dimensional speckle tracking layered strain technique in evaluating left ventricular systolic function in patients with systemic lupus erythematosus(SLE).Method Fifty SLE patients were selected as research object,and they were grouped into SLE group,and were evaluated by ultrasonic two-dimensional speckle tracking layered strain technique;In order to explore the diagnostic value of two-dimensional speckle tracking layered strain technique in SLE patients,another 50 healthy people were selected and divided into the control group.The results of echocardiography and global strain index were compared between the two groups.In addition,patients in the SLE group were grouped according to the course of disease.Patients with a disease course less than 3 years were classified into SLE I group(n=22),and patients with a disease course more than 3 years were classified into SLEⅡgroup(n=28).The results of echocardiography and the overall strain index were compared between the two groups.Results Compared with the control group,LVEF,LVEDd,PWT,LVFS and IVST in SLE group were not significantly different(P>0.05).Compared with the control group,GLS,GCS and LVTW were significantly different in SLE group(P<0.05).Compared with SLEⅡgroup,LVEF and LVFS in conventional echocardiographic indexes in SLEⅠgroup were significantly different(P<0.05);There was no significant difference in other indexes(P>0.05).Compared with SLEⅡgroup,GLS,GCS and LVTW indexes in SLEⅠgroup were significantly different(P<0.05).Conclusion SLE patients have different degrees of left ventricular function impairment,but it is difficult to detect it by conventional ultrasound.Through two-dimensional speckle tracking layered strain technology,there is a greater chance to detect the left ventricular impairment early,and then win the opportunity for patients to intervene early,which is helpful for patients to obtain a more ideal outcome.
