摘　　要：目的探讨血常规、生化、血清肿瘤标志物对皮肌炎的临床辅助诊断价值.方法收集皮肌炎患者90例(疾病组),并收集同期85名健康体检者(对照组)的临床信息,分析患者及健康体检者的基本信息,对相关血液学指标的水平差异进行分析,并通过二元Logistic回归比较单一指标和联合诊断的差异,做出相应的ROC曲线.结果疾病组细胞角质素片段19(CYFRA211)、神经元特异性烯醇化酶(NSE)、白细胞计数(WBC)水平较对照组显著升高(P<0.001).CYFRA211、NSE和WBC单独诊断皮肌炎时,ROC曲线下面积AUC均小于三者联合检测时的AUC,三者联合检测约登指数为0.729,敏感度和特异度分别为0.787、0.942,曲线下面积AUC为0.923.结论CYFRA211、NSE及WBC联合检测对皮肌炎具有一定的临床辅助诊断价值.Objective To explore the clinical diagnostic value of blood routine,biochemical and serum tumor markers in dermatomyositis.Method The total of 90 patients with dermatomyositis(disease group)and 85 healthy controls(control group)were collected.The basic information and the level differences of serum indicators were analyzed.Binary Logistic regression was used to compare the difference between single index and multiple index joint diagnosis,and the diagnostic efficiency was analyzed by ROC curve.Results The levels of CYFRA211,NSE and WBC in disease group were significantly higher than those in control group,and the differences were statistically significant(P<0.001).The area under ROC curve for single detection of CYFRA211,WBC and NSE was smaller than the area under ROC curve for cooperative detection of CYFRA211,WBC and NSE.The Yoden index of cooperative detection was 0.729,the sensitivity and specificity were 0.787 and 0.942,respectively.The area under ROC curve was 0.923.Conclusion The cooperative detection of CYFRA211,NSE and WBC has certain clinical auxiliary diagnostic value for dermatomyositis.
