窦口鼻道复合体变异与鼻中隔偏曲程度的相关性研究

摘　　要：目的研究窦口鼻道复合体(OMC)变异与鼻中隔偏曲程度的相关性。方法选取2016年6月至2020年6月在大同市第三人民医院接受鼻与鼻旁窦CT检查的848例患者,收集患者临床与影像资料并进行回顾性分析,由3名高年资主治医师对其检查结果进行诊断,根据鼻中隔偏曲诊断结果将患者分为无偏曲组(497例)、轻度偏曲组(146例)、中度偏曲组(122例)、重度偏曲组(83例),比较各组间OMC变异的发生率。结果中度及重度鼻中隔偏曲组OMC变异发生率分别为77.9%、87.9%,高于轻度偏曲组、无偏曲组的37.8%、45.9%(P<0.05),轻度偏曲组与无偏曲组间比较差异无统计学意义(P>0.05);Logistic单因素分析显示,鼻中隔中度及重度偏曲组发生OMC变异的风险高于无偏曲组(OR=1.847,P<0.05;OR=2.500,P<0.05);Logistic多因素因素分析显示,鼻中隔中度及重度偏曲组OMC变异发病风险高于无偏曲组(OR=1.578,P<0.05;OR=2.203,P<0.05)。结论中度、重度鼻中隔偏曲患者OMC变异患病率较无偏曲与轻度偏曲患者显著上升,可能与中、重度鼻中隔偏曲后鼻腔增宽有关。Objective To investigate the correlation between the anatomical variation of ostiomeatal complex(OMC)and varied severity of nasal septum deviation.Methods A total of 848 patients who under-went nasal and paranasal sinus CT in datong third people′s hospital between June 2016 and June 2020 were included and collected for clinical and imaging data for retrospective analysis.The CT findings were eval-uated by three senior physicians for the diagnosis.Depending on severity of the nasal septum deviation,the patients were divided into the non-deviation group(n=497),mild deviation group(n=146),moderate deviation group(n=122),and severe deviation group(n=83).The incidence of OMC variation was compared among groups.Results The incidence rates of OMC variation in the moderate and severe deviation groups were 77.9%and 87.9%,respectively,which were higher than those in the mild deviation group(37.8%)and non-deviation group(45.9%)(P<0.05),and did not vary statistically between the mild deviation group and non-deviation group(P>0.05).Univariate Logistic analysis showed that the risk of OMC variation was higher in the moderate and severe deviation groups compared with the non-deviation group(OR=1.847,P<0.05;OR=2.500,P<0.05).Multivariate Logistic analysis also revealed significantly higher risk of OMC variation in patients with moderate or severe deviation of nasal septum than in the non-deviation group(OR=1.578,P<0.05;OR=2.203,P<0.05).Conclusion Compared with mild or no deviation of nasal septum,moderate or severe deviation of nasal septum is more significantly correlated with OMC variation.This may be related to the greater compensatory effect of moderate and severe nasal septal deviation on nasal widening.
