青春期突发性失声的诊断与干预效果分析

摘　　要：目的探讨青春期突发性失声患者的诊疗方法及疗效。方法对4例青春期突发性失声患者的诊断、评估、治疗及疗效进行整理与分析。结果所有患者治疗前后的声学评估和主观听感知评估结果变化显著。患者的声压级和谐噪比较治疗前明显升高;基频、基频标准差、基频微扰、振幅微扰、清音比、嗓音中断比均较治疗前明显下降。治疗前所有患者G评分均为3分,治疗后均为0分。除患者甲外,所有患者的气息音和无力感评分均由3分降低至0分。治愈后3个月随访,所有患者均未复发。结论针对青春期突发性失声患者,嗓音行为学干预可以取得显著的治疗效果。Objective To investigate the diagnosis and treatment of adolescent patients with sudden aphonia.Methods The diagnosis,evaluation,treatment and curative effect of 4 cases of adolescent aphonia were analyzed.Results The results of acoustical assessment and subjective auditory perception assessment changed significantly in all patients before and after treatment.The SPL and HNR of patients were significantly higher than those before treatment.The fundamental frequency(F0),standard deviation of F0,jitter,shimmer,fraction of locally unvoiced frames,degree of voice breaks were significantly decreased compared with those before treatment.The G score of all patients was 3 points before treatment,and 0 points after treatment.The scores of Breathiness and Asthenia were reduced from 3 to 0 in all patients except patient A.After 3 months of follow-up,all patients had no recurrence.Conclusion For adolescent patients with sudden aphonia,voice behavior intervention can achieve significant therapeutic effect,and can be promoted in clinical diagnosis and treatment of this disease.
