摘 要:目的探讨在宫颈病变诊断中电子阴道镜引导下人乳头瘤病毒(HPV)和液基薄层细胞学检测(TCT)的作用。方法研究对象选取到我院妇科收治的早期宫颈病变的10186例患者,选取时间为2021年1月1日~2021年12月31日,对患者实施电子阴道镜指导下HPV和TCT,以病理活检结果作为确诊标准,对阴道镜指导下HPV和TCT检测以及HPV联合TCT检测的诊断价值进行评价。结果组织活检结果显示患者280例,CINⅠ、CINⅡ、CINⅢ患者分别有209例(74.64%)、17例(6.07%)和30例(10.71%),鳞状细胞癌5例(1.79%);在宫颈病变诊断中,HPV阳性率为52.31%,TCT阳性率为54.61%,HPV+TCT阳性率为60.77%;HPV和TCT诊阳性确率分别为8.07%和14.52%,组间对比无明显差异(P>0.05),HPV联合TCT诊断准确率为22.59%,明显高于HPV和TCT单一性检测(P<0.05)。结论在宫颈病变诊断中,电子阴道镜阴道下HPV和TCT检测有较高的诊断准确率,为了提高临床诊断准确率,避免误诊和漏诊,可以采取二者联合检测的方式。Objective To investigate the role of electronic colposcopy guided human papillomavirus(HPV)and liquid based thin layer cytology(TCT)in the diagnosis of cervical lesions.Methods The research objects were selected from 10186 patients with early cervical lesions admitted to the department of Gynecology of our hospital from January 1,2021 to December 31,2021.HPV and TCT were performed on the patients under the guidance of electronic colposcopy,and pathological biopsy results were used as diagnostic criteria.To evaluate the diagnostic value of HPV and TCT under colposcopy and HPV combined with TCT.Results Biopsy results showed that there were 280 patients,including 209(74.64%),17(6.07%)and 30(10.71%)CINⅰ,CINⅱand CINⅲ,respectively,and 5(1.79%)squamous cell carcinoma.The positive rate of HPV,TCT and HPV+TCT were 52.31%,54.61%and 60.77%respectively.The positive rates of HPV and TCT were 8.07%and 14.52%,respectively,and there was no significant difference between groups(P>0.05).The diagnostic accuracy of HPV combined with TCT was 22.59%,which was significantly higher than that of HPV and TCT alone(P<0.05).Conclusion In the diagnosis of cervical lesions,electronic colposcopy and vaginal HPV and TCT detection have high diagnostic accuracy.In order to improve the clinical diagnostic accuracy and avoid misdiagnosis and missed diagnosis,the combination of the two detection methods can be adopted.
