摘 要:目的 分析中性粒细胞/淋巴细胞(neutrophil/lymphocyte ratio, NLR)对糖尿病肾病(diabetes and kidney disease, DKD)肾功能不全的预测价值和影响。方法 回顾性分析2017年5月—2021年3月于漳州市长泰区妇幼保健院就诊治疗的121例DKD患者的临床资料,根据估算肾小球滤过率(e-GFR)结果,将患者分为肾功能不全组(52例)和非肾功能不全组(69例),分析NLR对DKD肾功能不全预测价值以及影响。结果 NLR预测DKD患者肾功能不全的AUC为0.735,最佳截断值为3.12。DKD肾功能不全组NLR≥3.12占比、病程及尿素氮(BUN)、肌酐(CR)、C-反应蛋白(CRP)、糖化血红蛋白(HbA1c)、血糖(GLU)水平均明显高于非肾功能不全组,e-GFR、ALB水平均明显低于非肾功能不全组,差异有统计学意义(P<0.05)。病程、NLR≥3.12、HbA1c均为DKD肾功能不全的独立影响因素(OR=1.451、2.010、3.149,P<0.05)。结论 NLR对DKD肾功能不全有一定预测价值,且为肾功能不全的独立影响因素。Objective To analyze the predictive value and effect of neutrophil/lymphocyte ratio(NLR)on renal insufficiency in diabetes and kidney disease(DKD).Methods The clinical data of 121 DKD patients treated in Changtai District Maternal and Child Health Hospital of Zhangzhou City from May 2017 to March 2021 were retrospectively analyzed.According to the estimated glomerular filtration rate(e-GFR)results,the patients were divided into renal insufficiency group(52 cases)and non-renal insufficiency group(69 cases),and the predictive value and impact of NLR on DKD renal insufficiency were analyzed.Results NLR predicted that the AUC of renal insufficiency in DKD patients was 0.735,and the best cutoff value was 3.12.The proportion of NLR≥3.12,the course of disease and the levels of BUN,creatinine(CR),C-reactive protein(CRP),glycosylated hemoglobin(HbA1c)and blood glucose(GLU)in DKD renal insufficiency group were significantly higher than those in non-renal insufficiency group,and the levels of e-GFR and ALB were significantly lower than those in non-renal insufficiency group,the difference was statistically significant(P<0.05).The course of disease,NLR≥3.12 and HbA1c were independent influencing factors of DKD renal insufficiency(OR=1.451,2.010,3.149,P<0.05).Conclusion NLR has a certain predictive value for renal insufficiency in DKD and is an independent influencing factor for renal insufficiency.
