摘 要:目的探讨血清胱抑素-C(Cys-C)、尿酸(UA)、尿免疫球蛋白G(U-IgG)联合检验在早期糖尿病肾病(DN)诊断中的应用价值。方法回顾性分析2021年3月—2022年4月浙江大学医学院附属第四医院收治的112例2型糖尿病患者,根据尿白蛋白/肌酐比值(UACR)分为对照组(UACR<30 mg/g)57例和研究组(30 mg/g≤UACR≤300 mg/g)55例,比较两组一般临床资料、肾功能指标及血清Cys-C、UA、U-IgG水平,采用Pearson相关及Logistic回归分析各指标与早期DN的关系,采用ROC曲线评估各指标对早期DN的诊断价值。结果与对照组相比,研究组Cys-C、UA、U-IgG水平明显升高,差异有统计学意义(P<0.05),Pearson相关分析显示,血清Cys-C、UA、U-IgG与血清尿素氮(BUN)、肌酐(Scr)均呈正相关,与估计肾小球滤过率(eGFR)均呈负相关;Logistic回归分析表明,Cys-C、UA、U-IgG是早期DN的独立危险因素(P<0.05);ROC曲线分析结果显示,UIgG对早期DN有较高的诊断价值,三者联合检验的曲线下面积最大,敏感度、特异度均较高。结论血清Cys-C、UA、U-IgG均与糖尿病早期肾功能损害相关,是早期DN的独立危险因子,三者联合检测对早期DN具有较高的诊断价值。Objective To investigate the application value of serum cystatin C(Cys-C),uric acid(UA)and urinary immunoglobulin G(U-IgG)combined test in the diagnosis of early diabetic nephropathy(DN).Methods One hundred and twelve patients with type 2 diabetes admitted to the Fourth Affiliated Hospital of Zhejiang University School of Medicine from March 2021 to April 2022 were retrospectively analyzed.According to urinary albumin/creatinine ratio(UACR),fifty-seven cases were divided into control group(UACR<30 mg/g)and fifty-five cases were divided into study group(30 mg/g≤UACR≤300 mg/g).General clinical data,renal function indexes and serum levels of Cys-C,UA and U-IgG were examined and compared between the two groups.Pearson correlation and Logistic regression were used to analyze the relationship between each index and early DN,and ROC curve was used to evaluate the diagnostic value of each index for early DN.Results Compared with the control group,the levels of Cys-C,UA and U-IgG in the study group were significantly increased,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that serum Cys-C,UA and U-IgG were positively correlated with serum urea nitrogen(BUN)and creatinine(Scr),and negatively correlated with estimated glomerular filtration rate(eGFR).Logistic regression analysis showed that Cys-C,UA and U-IgG were independent risk factors for early DN(P<0.05).ROC curve analysis results showed that U-IgG had a high diagnostic value for early DN,and the combined test of the three had the largest area under the curve,with high sensitivity and specificity.Conclusion Serum Cys-C,UA and U-IgG are all associated with renal function impairment in the early stage of diabetes and are independent risk factors for early DN.The combined detection of the three is of high diagnostic value for early DN.
