精细化护理对高龄糖尿病肾病血液透析患者负面情绪与肾功能的影响分析

摘　　要：目的 研究精细化护理对高龄糖尿病肾病血液透析患者负面情绪与肾功能的影响。方法 选取2020年7月—2022年7月福建医科大学附属三明市第一医院肾内科收治的60例高龄糖尿病肾病血液透析患者,按照随机数表方法分为对照组(30例)和观察组(30例)。对照组患者采用常规护理,观察组患者采用精细化护理,对比两组患者在不同护理方法下的血糖水平、护理满意度、负面情绪评分、临床指标、自我护理能力评分、不良反应发生情况。结果 观察组患者护理后空腹血糖、餐后2 h血糖、糖化血红蛋白均低于对照组,差异有统计学意义(t=12.845、12.970、5.860,P<0.05)。观察组总不良反应发生率低于对照组,差异有统计学意义(χ^(2)=7.124,P<0.05)。结论 精细化护理对高龄糖尿病肾病血液透析患者负面情绪与肾功能具有积极影响,具推广价值。Objective To study the effect of intensive nursing on negative emotion and renal function in elderly patients with diabetic nephropathy and hemodialysis.Methods Sixty elderly diabetic nephropathy patients admitted to the Department of Nephrology,Sanming First Hospital Affiliated to Fujian Medical University from July 2020 to July 2022 were selected and divided into control group(30 cases)and observation group(30 cases)according to random number table method.The control group received conventional nursing care,and the observation group received refined nursing care.Compare the blood glucose levels,nursing satisfaction,negative emotion scores,clinical indicators,self-care ability scores,and incidence of adverse reactions between two groups of patients under different nursing methods.Results The fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin of patients in the observation group were lower than those in the control group,and the difference was statistically significant(t=12.845,12.970,5.860,P<0.05).The total incidence of adverse reactions in the observation group were lower than those in the control group,and the difference was statistically significant(χ^(2)=7.124,P<0.05).Conclusion Refined nursing has positive effects on negative emotions and renal function in elderly patients with diabetic nephropathy on hemodialysis,and it is more valuable to popularize.
