摘　　要：目的 探究分析糖尿病人群常规筛查中糖尿病视网膜病变的影响因素。方法 选取2020年6月—2022年6月江南大学附属医院内分泌科住院治疗且进行早期糖尿病性视网膜病变筛查的1 034例糖尿病患者为研究对象,根据筛查结果分为并发糖尿病性视网膜病变组(172例)和未并发糖尿病性视网膜病变组(862例)。分析糖尿病性视网膜病变的主要影响因素。结果 单因素分析中,两组患者文化程度、病程、糖化血红蛋白、空腹血糖、餐后2 h血糖、血尿酸指标水平对比,差异有统计学意义(P<0.05)。经多因素Logistic回归分析后,结果表明FBG(OR=2.307)、病程(OR=2.214)、文化程度(OR=2.250),Hb Alc(OR=2.361)均为独立危险因素(P<0.05)。结论 糖尿病性视网膜病变的主要影响因素包括FBG、病程、文化程度、Hb Alc在临床中治疗中针对血糖水平控制效果差、文化水平低、糖尿病病程时间长的患者给予重点关注,预防糖尿病性视网膜病变的发生。Objective To explore and analyze the influencing factors of diabetic retinopathy in routine screening of diabetic population.Methods 1034 patients with diabetes who were hospitalized in the Department of Endocrinology of the Affiliated Hospital of Jiangnan University from June 2020 to June 2022 and screened for early diabetes retinopathy were selected as the study objects.According to the screening results,they were divided into the group with diabetes retinopathy(172 cases)and the group without diabetes retinopathy(862 cases).To analyze the main influencing factors of diabetes retinopathy.Results In univariate analysis,there were statistical differences between the two groups in terms of education level,course of disease,glycosylated hemoglobin(HbAlc),fasting blood glucose(FBG),postprandial blood glucose(PBG),and uric acid(UA)(P<0.05).After multivariate logistic regression analysis,the results showed that FBG(OR=2.307),course of disease(OR=2.214),education level(OR=2.250)and HbAlc(OR=2.361)were independent risk factors(P<0.05).Conclusion The main influencing factors of diabetes retinopathy include FBG,course of disease,education level and HbAlc.In clinical treatment,patients with poor blood sugar control effect,low education level and long course of diabetes should be given special attention,so as to prevent the occurrence of diabetes retinopathy.
