Effectiveness of Biologic Femoral Long-stem Hemi-arthroplasty in Inter⁃trochanteric Fracture of Femur

摘  要:目的探讨生物型股骨长柄半髋关节置换在股骨粗隆间骨折中的治疗效果。方法方便选取2019年5月—2021年3月陇南市第一人民医院收治的股骨粗隆间骨折患者92例,随机分为对照组(n=46)和研究组(n=46),对照组实施股骨近端防旋髓内钉(PFNA)内固定术,研究组实施生物型股骨长柄半髋关节置换治疗。比较两组炎性反应因子水平、相关指标、Harris评分、VAS评分。结果研究组IL-10、IL-6、TNF-α水平为(37.53±3.11)、(42.82±5.43)、(40.43±3.38)pg/mL水平低于对照组(44.66±4.33)、(51.82±6.22)、(48.17±4.05)pg/mL,差异有统计学意义(t=9.071、7.393、9.951,P<0.001);研究组骨折愈合时间、下地负重时间、住院时间、术中出血量、手术时间(17.47±2.88)周、(56.28±4.27)d、(17.36±2.42)d、(213.28±23.72)mL、(80.37±4.33)min低于对照组(21.54±2.96)周、(68.73±5.12)d、(21.93±2.53)d、(357.47±30.62)mL、89.43±5.57)min,差异有统计学意义(t=6.684、12.666、8.853、25.249、8.710,P<0.001);研究组Harris评分高于对照组,差异有统计学意义(t=-9.796,P<0.001),VAS评分分低于对照组分,差异有统计学意义(t=19.093,P<0.001)。结论在股骨粗隆间骨折中,生物型股骨长柄半髋关节置换的治疗效果理想,可有效改善Harris评分。Objective To investigate the therapeutic effect of biologic femoral long-stem hemi-arthroplasty in intertro⁃chanteric fracture of femur.Methods 92 patients with intertrochanteric fractures of the femur admitted to The First People's Hospital of Longnan City from May 2019 to March 2021 were selected and randomly divided into a control group(n=46)and a study group(n=46).The control group received proximal femoral anti rotation intramedullary nail(PFNA)internal fixation,while the study group received biological femoral long stem half hip replacement treatment.Compare the levels of inflammatory response factors,relevant indicators,Harris scores,and VAS scores between the two groups.Results The levels of IL-10,IL-6,TNF-αin research group(37.53±3.11)pg/mL,(42.82±5.43),and(40.43±3.38)pg/mL were lower than those of the control group(44.66±4.33)pg/mL,(51.82±6.22)pg/mL,and(48.17±4.05)pg/mL with statistically significant differences(t=9.071,7.393,9.951,P<0.001);the fracture healing time,weight-bearing time,hospitalization time,intraoperative bleeding volume,surgical time(17.47±2.88)weeks,(56.28±4.27)d,(17.36±2.42)d,(213.28±23.72)mL,and(80.37±4.33)min in the study group were lower than those in the control group(21.54±2.96)weeks,(68.73±5.12)d,(21.93±2.53)d,(357.47±30.62)mL,and(89.43±5.57)min,and the difference was statistically significant(t=6.684,12.666,8.853,25.249,8.710,P<0.001);the Harris score of the re⁃search group was higher than that of the control group,and the difference was statistically significant(t=-9.796,P<0.001),while the VAS score was lower than that of the control group,and the difference was statistically significant(t=19.093,P<0.001).Conclusion In intertrochanteric fracture of femur,the treatment effect of biotype femoral long-stem hemi-arthroplasty is ideal and can effectively improve the Harris score,which should be further promoted in clinical practice.
