摘 要:目的:分析不同剂量左旋甲状腺素钠治疗新生儿先天性甲状腺功能减低症(CH)的临床效果。方法:选取2017年9月—2021年9月苍南县人民医院确诊的CH患儿100例为观察对象,根据用药的初始剂量分为治疗1组与治疗2组,各50例。治疗1组左甲状腺素初始常规剂量为6~10μg·kg^(-1)·d^(-1),治疗2组左甲状腺素初始低剂量为1~5μg·kg^(-1)·d^(-1)。比较两组患儿甲状腺功能指标、治疗效果、发育情况。结果:治疗后,两组患儿游离甲状腺素(FT4)水平均升高,促甲状腺激素(TSH)水平均降低,差异有统计学意义(P<0.05);治疗1组FT4水平高于治疗2组,TSH水平低于治疗2组,差异有统计学意义(P<0.001);治疗1组治疗总有效率高于治疗2组,差异有统计学意义(P=0.017);治疗后,两组患儿身高、体重均升高,治疗1组高于治疗2组,差异有统计学意义(P<0.05);治疗1组患儿DQ值>115例数高于治疗2组,治疗1组患儿DQ值≤84例数低于治疗2组,差异具有统计学意义(P<0.05)。结论:左旋甲状腺素钠治疗CH患儿中,采取大剂量治疗可获得明显治疗效果,能够促进患儿的生长发育,改善患儿甲状腺功能。Objective:To analyze the clinical effects of different doses of levothyroxine sodium in the treatment of neonatal congenital hypothyroidism(CH).Methods:From September 2017 to September 2021,100 children with CH diagnosed in Cangnan County People's Hospital were selected as observation objects.According to the initial dose of medication,they were divided into treatment group 1 and treatment group 2,with 50 cases in each group.The usual initial dose of levothyroxine in treatment group 1 was 6~10μg·kg^(-1)·d^(-1).The initial low dose of levothyroxine in treatment group 2 was 1~5μg·kg^(-1)·d^(-1).The thyroid function indexes,treatment effect and development were compared between the two groups.Results:After treatment,the levels of free thyroxine(FT4)increased and the levels of thyroid-stimulating hormone(TSH)decreased in both groups,and the difference was statistically significant(P<0.05).The FT4 level in the treatment group 1 was higher than that in the treatment group 2,and the TSH level was lower than that in the treatment group 2,and the difference was statistically significant(P<0.001).The total effective rate of treatment group 1 was higher than that of treatment group 2,and the difference was statistically significant(P=0.017).After treatment,the height and weight of the children in the two groups increased,and the treatment group 1 was higher than the treatment group 2,and the difference was statistically significant(P<0.05).The number of children with DQ value>115 in treatment group 1 was higher than that in treatment group 2,and the number of children with DQ value≤84 in treatment group 1 was lower than that in treatment group 2,and the difference was statistically significant(P<0.05).Conclusion:In the treatment of CH children with levothyroxine sodium,high-dose treatment can obtain obvious therapeutic effect,can promote the growth and development of children,and improve thyroid function in children.
