摘 要:目的:探讨超声心动图对不同期职业性尘肺患者肺动脉高压和右心功能的评估效果。方法:选取2019年1月—2020年12月广西壮族自治区工人医院收治的职业性尘肺患者126例为观察对象,根据不同临床分期分为壹期组51例、贰期组43例、叁期组32例,另选取同期健康对照组35例。研究对象均进行超声心动图检查,比较4组肺动脉高压和右心功能的测量数据。结果:叁期组三尖瓣环收缩期位移(TAPSE)、右心室面积变化分数(RVFAC)低于壹期组、贰期组、对照组,三尖瓣环收缩期峰值速度(S')、估测肺血管阻力(PVR)、右心室舒张末期横径/左心室舒张末期横径(RVETD/LVETD)、肺动脉收缩压(PASP)高于壹期组、贰期组、对照组,差异有统计学意义(P=0.001)。结论:超声心动图可准确监测患者TAPSE、RVFAC、S'、PVR、RVETD/LVETD、PASP指标,对于不同期职业性尘肺患者临床诊疗评价和病情监测有重要参考价值。Objective:To explore the evaluation effect of echocardiography on pulmonary hypertension and right heart function in patients with occupational pneumoconiosis at different stages.Methods:From January 2019 to December 2020,one hundred and twenty-six cases of occupational pneumoconiosis patients admitted to Workers Hospital of Guangxi Zhuang Autonomous Region were selected as observation objects.According to different clinical stages,they were divided into the first-stage group with 51 cases,the second-stage group with 43 cases,and the third-stage group with 32 cases.Another 35 cases of healthy control group were selected in the same period.All subjects underwent echocardiography,and the measurement data of pulmonary hypertension and right heart function were compared among the four groups.Results:The tricuspid annulus systolic excursion(TAPSE)and right ventricular area fractional change(RVFAC)in the third-stage group were lower than those in the first-stage group,the second-stage group,and the control group.Tricuspid annular systolic peak velocity(S'),estimated pulmonary vascular resistance(PVR),right ventricular end-diastolic diameter/left ventricular end-diastolic diameter(RVETD/LVETD),pulmonary artery systolic pressure(PASP)higher than first-stage group,the second-stage group,and control group.The difference was statistically significant(P=0.001).Conclusion:Echocardiography can accurately monitor TAPSE,RVFAC,S',PVR,RVETD/LVETD,and PASP indicators in patients,and has important reference value for clinical diagnosis and treatment evaluation and condition monitoring of occupational pneumoconiosis patients at different stages.
