“见肝之病,知肝传脾”在乳腺癌全程管理中的应用

摘　　要：“见肝之病,知肝传脾”是张仲景在《金匮要略》提出的诊疗方法,其中蕴含了“治未病”思想。乳腺癌全程管理是近期现代医学提出的概念,主要目的为预防复发转移和提高生活质量。中医药在乳腺癌的术后期、化疗期、放疗期、内分泌治疗期、靶向治疗期和维持治疗期都发挥了重要的作用,术后期主要体现在治疗术后并发症,如疲乏、上肢淋巴水肿;放、化疗期重在预防和治疗放疗、化疗的不良反应,用药根据化疗方案和放疗部位的不同有所侧重;靶向治疗期重在预防和治疗靶向药的不良反应以及预防复发转移;维持治疗期重在防复发、防转移,中医药防治主要从调整体质、清除余毒和顾护易转移脏腑正气三方面入手。在现代医学的全程管理过程中,中医经典理论是全程管理概念的内核之一。The therapeutic method of“disease in liver may transmit from liver to spleen”was proposed by ZHANG Zhongjing in Synopsis of Golden Chamber,which contains the idea of“prevention before disease onset”.The concept of whole course management of breast cancer is a recent concept proposed by modern medicine,the main purpose of which is to prevent recurrence and metastasis,and improve the quality of life.Chinese medicine plays an important role in the postoperative period,chemotherapy period,radiotherapy period,endocrine therapy period,targeted therapy period and maintenance therapy period of breast cancer.During the postoperative period,Chinese medicine is mainly used for the treatment of postoperative complications,such as fatigue and lymphedema of the upper limbs.During the radiotherapy and chemotherapy periods,it focuses on the prevention and treatment of the adverse effects of radiotherapy and chemotherapy,and the medicines used are focused on different chemotherapy regimens and radiotherapy sites.During the targeted therapy period,it focuses on prevention and treatment of adverse reactions of targeted drugs and prevention of recurrence and metastasis.During the maintenance treatment period,the main focus is on prevention of recurrence and metastasis.The prevention and treatment of Chinese medicine mainly starts from three aspects:adjusting body constitution,removing residual toxicity and caring for the healthy qi of the vulnerable Zang-fu organs.The classical theory of traditional Chinese medicine is one of the core concepts of the whole course management in modern medicine.
