阴道上皮内瘤变与TCT、高危型HPV感染、宫颈病变的关系研究

摘　　要：目的探讨阴道上皮内瘤变(VAIN)与液基细胞学(TCT)、高危型人乳头瘤病毒(HR-HPV)感染及宫颈病变的关系。方法选取120例VAIN患者,对患者临床资料进行整理,分析该疾病的发生与TCT、高危型HR-HPV感染及宫颈病变的关系。结果120例患者中,≥42岁患者发生VAIN的占比均明显高于<42岁者;HR-HPV感染阳性者共计106例,占比88.33%,低度鳞状上皮内瘤变(ISIL)与高度鳞状上皮内瘤变(HSIL)的HR-HPV阳性率比较,差异无统计学意义(P>0.05)。随着VAIN级别的提升,TCT诊断级别也不断提升;在VAIN患者中,有78.33%的患者出现宫颈病变(94/120),患者的VAIN级别随着宫颈病变级别的升高而逐渐升高。结论VAIN的发病与年龄存在密切的关系,其病情严重程度与TCT、HR-HPV感染及宫颈病变级别存在一定关系。Objective To investigate the relationship between vaginal intraepithelial neoplasia(VAIN)and Thinprep cytologic test(TCT),human papillomavirus(HR-HPV)infection and cervical lesions.Methods Clinical data of 120 VAIN patients were selected to analyze the relationship between the occurrence of the disease and TCT,high-risk HR-HPV infection and cervical lesions.Results Among 120 patients,the proportion of VAIN in patients≥42 years old was significantly higher than that in patients<42 years old.A total of 106 cases were positive for HR-HPV infection,accounting for 88.33%.There was no significant difference in the positive rate of HR-HPV between low-grade squamous intraepithelial neoplasia(ISIL)and high-grade squamous intraepithelial neoplasia(HSIL)(P>0.05).With the increase of VAIN level,the TCT diagnosis level also increased.Cervical lesions occurred in 78.33%of VAIN patients(94/120).Studies showed that the level of VAIN increased with the increase of the level of cervical lesions.Conclusion The incidence of VAIN was closely related to age,and the severity of VAIN was related to TCT,HR-HPV infection and the degree of cervical lesions.
