摘 要:目的探讨重症颅脑损伤与心脏功能损伤指标的相关性。方法选取2018年12月至2020年12月广州市番禺区何贤纪念医院收治的重症颅脑损伤患者42例作为观察组,另外选取同期体检科的体检健康者20名作为对照组,观察组入院后抽取患者静脉血,对照组取体检血样,检查两组的肿瘤坏死因子-α(TNF-α)、磷脂酶A2(PLA2)、心肌肌钙蛋白T(cTnT)及肌酸激酶同工酶(CK-MB)水平。对两组检查结果进行比较,并对观察组患者检查结果的相关性进行分析。结果观察组患者的cTnT、CK-MB水平高于对照组,差异有统计学意义(P<0.05);观察组患者的TNF-α、PLA2水平高于对照组,差异有统计学意义(P<0.05);Spearman相关分析结果显示,PLA2与cTnT、CK-MB呈正相关(r=0.756,P<0.001;r=0.825,P<0.001),TNF-α与cTnT、CK-MB呈正相关(r=0.894,P<0.001;r=0.915,P<0.001)。结论TNF-α、PLA2水平与重症颅脑损伤后心脏功能损害的发生存在密切关系。Objective To investigate the correlation between severe craniocerebral injury and indexes of cardiac function damage.Methods A total of 42 patients with severe craniocerebral injury admitted to Guangzhou Panyu Hexian Memorial Hospital from December 2018 to December 2020 were selected as the observation group,and 20 healthy people in the physical examination department during the same period were selected as the control group.Venous blood was collected from the patients in the observation group after admission.Blood samples were taken from the control group.The levels of tumor necrosis factor-α(TNF-α),phospholipase A2(PLA2),cardiac troponin T(cTnT)and creatine kinase isoenzyme(CK-MB)of the two groups were detected.The examination results of the two groups were compared,and the correlation between the examination results of the observation group was analyzed.Results The cTnT and CK-MB levels of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The TNF-αand PLA2 levels of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that PLA2 was positively correlated with cTnT and CK-MB(r=0.756,P<0.001;r=0.825,P<0.001).TNF-αwas positively correlated with cTnT and CK-MB(r=0.894,P<0.001;r=0.915,P<0.001).Conclusion The levels of TNF-αand PLA2 are closely related to the occurrence of cardiac function damage after severe craniocerebral injury.
