血清白介素-6、淀粉样蛋白A联合降钙素原对儿童社区性获得性肺炎的诊断价值研究

摘　　要：目的探讨血清白介素-6(IL-6)、淀粉样蛋白A(SAA)、降钙素原(PCT)联合检测在儿童社区获得性肺炎(CAP)中的诊断效能。方法将2021年1月至2022年11月江西省新余市妇幼保健院收治的62例儿童CAP患儿纳入观察组,同期行体检的65名健康儿童纳入对照组,采集两组儿童静脉血5 ml,检测比较两组的IL-6、SAA、PCT的差异;绘制ROC曲线,分析IL-6、SAA、PCT单独与联合检测在儿童CAP中的诊断效能。结果观察组的IL-6为(20.63±2.41)pg/ml、SAA为(12.53±2.35)mg/L、PCT为(1.35±0.29)pg/ml,均高于对照组的(3.48±0.51)pg/ml、(5.31±0.48)mg/L、(0.33±0.05)pg/ml,差异有统计学意义(P<0.05)。ROC曲线结果显示,IL-6、SAA、PCT三项指标联合检测在儿童CAP诊断中的AUC为0.931(95%CI:0.886~0.983),高于三项指标单独检测。结论IL-6、SAA、PCT在儿童CAP患儿的血清内呈明显的高表达状态,三项指标联合检测可有效提高疾病的检出准确率,可为临床提供可靠的参考依据,可进行大力的推行。Objective To investigate the diagnostic efficacy of serum interleukin-6(IL-6),amyloid A(SAA)and procalcitonin(PCT)in children with community acquired pneumonia(CAP).Methods From January 2021 to November 2022,62 children with CAP admitted to Xinyu Maternal and Child Health Hospital in Jiangxi Province were included in the observation group,and 65 healthy children who underwent physical examination were included in the control group.5 ml of venous blood was collected from the two groups,and the differences of IL-6,SAA and PCT between the two groups were detected and compared.In addition,the ROC was drawn to analyze the diagnostic efficacy of IL-6,SAA and PCT alone and jointly in children with CAP.Results The levels of IL-6,SAA and PCT in the observation group were(20.63±2.41)pg/ml,(12.53±2.35)mg/L and(1.35±0.29)pg/ml respectively,which were higher than those in the control group(3.48±0.51)pg/ml,(5.31±0.48)mg/L and(0.33±0.05)pg/ml,the differences were statistically significant(P<0.05).ROC results showed that the AUC of the combined detection of IL-6,SAA and PCT in children's CAP diagnosis was 0.931(95%CI:0.886-0.983),which was higher than that of the independent detection of the three indicators.Conclusion IL-6,SAA and PCT are highly expressed in the serum of children with CAP.The combined detection of these three indicators can effectively improve the accuracy of disease detection,provide reliable reference for clinical practice,and can be vigorously implemented.
