黄苓泽泻合剂治疗湿浊内蕴型高尿酸血症的效果及对炎症应激反应、肾功能的影响

摘　　要：目的分析黄苓泽泻合剂治疗湿浊内蕴型高尿酸血症(HUA)的效果及对炎症应激反应、肾功能的影响。方法选取2020年1月至2022年1月青岛市黄岛区中医医院内分泌科的96例湿浊内蕴型HUA患者作为研究对象,采用随机数字表法将其分为试验组与基础组,每组各48例。基础组采用常规西药治疗,试验组在基础组的基础上采用黄苓泽泻合剂治疗。比较两组患者的临床疗效、中医证候积分、炎症应激反应指标、肾功能指标、不良反应总发生率。结果试验组患者的临床总有效率(95.83%)高于基础组(72.92%),差异有统计学意义(P<0.05)。试验组患者治疗后的腰部酸痛、肢体困重、活动受限积分均低于基础组,差异有统计学意义(P<0.05)。试验组患者治疗后的血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均低于基础组,差异有统计学意义(P<0.05)。试验组患者治疗后的血清胱抑素C(CysC)、尿酸(UA)、肌酐(SCr)均低于基础组,差异有统计学意义(P<0.05)。试验组患者的不良反应总发生率(4.17%)与基础组(8.33%)比较,差异无统计学意义(P>0.05)。结论黄苓泽泻合剂可有效缓解湿浊内蕴型HUA患者临床症状,改善肾功能,减轻炎症应激反应,且治疗期间患者未出现明显不良反应,具有一定的安全性。Objective To analyze the efficacy of Huangling Zexie Mixture in the treatment of hyperuricemia(HUA)due to internal accumulation of dampness and turbidity and its effect on inflammatory stress response and renal function.Methods From January 2020 to January 2022,96 patients with hyperuricemia due to internal accumulation of dampness and turbidity in the Department of Endocrinology,Traditional Chinese Medicine Hospital of Huangdao District,Qingdao were included in the research.They were divided into experimental group and basic group by random number table,with 48 cases in each group.The basic group was treated with conventional western medicine,and the experimental group was treated with Huangling Zexie Mixture on the basis of basic group.The clinical efficacy,traditional Chinese medicine syndrome score,inflammatory stress response index,renal function index and total incidence of adverse reactions were compared between the two groups.Results The total clinical effectiveness rate of the experimental group was 95.83%,higher than that of the control group(72.92%),and the difference was statistically significant(P<0.05).After treatment,the scores of aching pain in the lower back,limb heaviness,and activity limitation in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the serum levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).The serum levels of cystatin C(CysC),uric acid(UA)and creatinine(SCr)in the experimental group after treatment were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the experimental group(4.17%)and the control group(8.33%)(P>0.05).Conclusion Huangling Zexie Mixture can effectively relieve the clinical symptoms,improve renal functi
