关节镜下辅助拇长屈肌腱转位治疗陈旧性跟腱断裂的临床研究

摘　　要：目的:探究使用关节镜辅助下全长拇长屈肌腱转位术治疗陈旧性跟腱断裂的临床效果。方法:回顾性分析2019年5月至2022年2月云浮市郁南县中医院收治的32例陈旧性跟腱断裂患者的临床资料。将其分为内镜手术组和常规切开组。内镜手术组12例患者采用关节镜辅助下全长拇长屈肌腱转位术进行治疗,常规切开组20例患者采用传统切开非全长拇长屈肌腱转位手术进行治疗。比较两组的术后镇痛率、并发症发生率、术中出血量、手术时间等围术期指标以及VISA-A(维多利亚运动跟腱评分)、AOFAS(美国足踝学会后足评分)、ATRS(跟腱断裂评分)等功能指标情况。结果:内镜手术组的各项围术期指标均优于常规切开组(P<0.05)。治疗前,两组的各项功能指标相比无显著差异(P>0.05)。治疗后,内镜手术组的各项功能指标均优于常规切开组(P<0.05)。结论:关节镜辅助下全长拇长屈肌腱转位术治疗陈旧性跟腱断裂的临床效果显著,具有手术创伤小、患者舒适度高、功能恢复速度快等优势,临床推广价值较高。Objective:To explore the clinical effect of arthros copically assisted full-length flexor pollicis longus tendon transposition in the treatment of old Achilles tendon rupture.Methods:The clinical data of 32 patients with old Achilles tendon rupture admitted to Yunan Hospital of Traditional Chinese Medicine in Yunfu City from May 2019 to February 2022 were retrospectively analyzed.The patients were divided into endoscopic surgery group and conventional incision group.Twelve patients in the endoscopic surgery group were treated with arthroscopic assisted full-length flexor pollicis longus tendon transposition,and 20 patients in the conventional incision group were treated with traditional open non-full-length flexor pollicis longus tendon transposition.Postoperative analgesia rate,complication rate,intraoperative blood loss,operative time and other perioperative indexes,as well as functional indexes such as VISA-A(Victoria Sports Achilles Tendon score),AOFAS(American Foot and Ankle Society Posterior foot score)and ATRS(Achilles tendon rupture score)were compared between the two groups.Results:All perioperative indexes of endoscopic surgery group were better than those of conventional incision group(P<0.05).Before treatment,there were no significant differences in various functional indexes between the two groups(P>0.05).After treatment,the functional indexes of the endoscop ic surgery group were better than those of the conventional incision group(P<0.05).Conclusion:Arthroscopic assisted full-length flexor hallucis longus tendon transposition in the treatment of old Achilles tendon rupture has significant clinical effect,with advantages of less surgical trauma,high comfort of patients,fast functional recovery,etc.,and has high clinical promotion value.
