摘  要:目的:探讨二维灰阶超声肝包膜形态、肝实质回声分类与肝脏剪切波弹性值之间的关系及其在肝纤维化分期中的应用。方法:回顾研究2018年6月-2021年6月在广东省第二中医院就诊的166例乙肝小三阳患者,收集患者的二维灰阶超声下肝包膜形态、肝实质回声评价结果及肝剪切波弹性值,再分组进行统计分析。结果:按肝包膜形态、肝实质回声分组发现,组间剪切波弹性值的差异有统计学意义,且肝包膜形态、肝实质回声分组间存在一定的相关性。结论:二维灰阶超声显示的不同肝包膜形态、肝实质回声对应着不同的剪切波弹性值范围,据此可以进行乙肝肝纤维化分期。Objective:To investigate the relationship between the morphology of liver capsule,echo classification of liver parenchyma and the elastic value of liver shear wave by two-dimensional gray scale ultrasound and its application in the staging of liver fibrosis.Methods:A retrospective study was conducted on 166 patients with hepatitis B and minor three Yang treated in the Second Middle School Hospital of Guangdong Province from June 2018 to June 2021.Liver capsule morphology,liver parenchyma echo evaluation results and liver shear wave elasticity values of patients were collected under two-dimensional gray scale ultrasound,and then grouped for statistical analysis.Results:According to liver capsule morphology and liver parenchymal echo grouping,the difference of shear wave elasticity between groups was statistically significant,and there was a certain correlation between liver capsule morphology and liver parenchymal echo grouping.Conclusion:Different morphology of liver envelope and echo of liver parenchyma showed by two-dimensional gray scale ultrasound correspond to different elastic values of shear wave,which can be used to stage hepatitis B liver fibrosis.
