摘 要:随着重症监护技术的进步和标准化护理的广泛应用,近年来脓毒症患者死亡率正在逐步下降,而脓毒症的发病率仍呈逐年上升的趋势。脓毒症是急诊常见的致死、致残性危重病,对世界各地的卫生保健系统构成了重大挑战。在脓毒症3.0中,脓毒症被定义为宿主对感染反应失调而致危及生命的器官功能障碍。无论病因如何,一旦发现脓毒症性休克,每延迟1 h的抗生素治疗,存活率就会下降7.6%。早期识别感染的原因和脓毒症患者的预后分层是重要的临床优先事项。早期诊断可显著提高脓毒症患者救治的成功率,但目前早期诊断的方法仍存在一定的局限性。microRNA(miRNA)是19~22个碱基的短单链RNA分子,可以通过碱基配对结合mRNA,导致靶标被破坏或抑制mRNA蛋白质翻译。血液中的miRNA可作为潜在的生物标志物,并为疾病的治疗干预提供靶点。本文旨在总结miRNA在脓毒症关键性阶段所起的调控性作用,并评价miRNA作为一类新的诊断标志物与治疗靶点在脓毒症中的应用价值。With the progress of intensive care technology and the wide application of standardized nursing,the mortality rate of sepsis patients is gradually decreasing in recent years.However,the morbidity of sepsis patients is still increasing year by year.Sepsis is a common fatal and disability-related critical disease in emergency care,which poses a major challenge to health care systems around the world.In sepsis 3.0,sepsis is defined as a life-threatening organ dysfunction caused by the parasitifer's dysfunction to infection.There is evidence that,regardless of the causes,once sepsis shock is found,the survival rate will decrease by 7.6%for every hour of delayed antibiotic treatment.Early identification of the causes of infection and prognostic stratification of patients with sepsis are important clinical priorities.Early diagnosis can significantly improve the treatment success rate of sepsis patients,but there are still certain limitations in early diagnosis methods.microRNA is a short single-stranded RNA molecule of 19-22 bases,which can bind to mRNA by base pairing,resulting in target destruction or inhibiting the protein translation of the mRNA.mRNA in the blood may serve as potential biomarkers and provide opportunities for treatment interventions of diseases.This article aims to summarize the regulatory role of miRNA in the critical stage of sepsis and evaluate the application value of miRNA as a new diagnostic marker and therapeutic target in sepsis.
