中晚期肺癌患者心理困扰与感知社会约束的相关性及对策

摘　　要：目的调查中晚期肺癌患者心理困扰及其与感知社会约束的相关性及制订对策,为减轻癌症患者心理困扰提供依据。方法采用便利抽样法,选取本院2020年1月至2022年10月收治的中晚期肺癌患者120例,采用一般资料调查表、心理困扰筛查工具(screening tools for measure distress,DM)和社会约束量表((social constraints scale-15,SCS-15)进行调查。结果120例中晚期患者心理困扰平均(4.05±1.20)分,发生率60.00%,中重度的首要问题是经济压力(70.83%,85/120),其次是疾病相关信息不足(69.17%,83/120),排第3为给家人造成负担(63.33%,76/120)。患者感知社会约束得分(33.50±6.92)分,与心理困扰呈正相关(P<0.05);Logistic回归分析显示家庭月收入(<3000元)、疼痛、感知社会约束是心理困扰发生的独立危险因素(均P<0.05),其共同解释总变异的42.50%。结论中晚期肺癌患者心理困扰处于中等程度,与感知社会约束密切相关,通过减轻患者感知社会约束、疼痛等可改善其心理困扰。Objective To investigate the correlations between psychological distress and perceived social constraint in patients with intermediate and advanced lung cancer in order to provide a basis for reduction of psychological distress in patients with lung cancer.Methods A total of 120 patients with intermediate and advanced lung cancer hospitalised in Beijing Chaoyang Hospital of Capital Medical University from January 2020 to December 2022 were selected by convenient sampling method.This study was conducted by the methods of general data questionnaire,screening tools for measure distress,(DM)and social constraints scale-15(SCS-15).Results The mean score of distress thermometer(DT)was(4.05±1.20),with an occurrence rate of 60.00%.For the patients,the most serious problem was financial stress(70.83%,85/120),followed by lack of information about diseases(69.17%,83/120)and burden on family members(63.33%,76/120).SCS-15 score of patients was(33.50±6.92)and it was positively correlated with DT score(P<0.05)by Pearson correlation.Logistic regression analysis showed that monthly family income(<3000 yuan CNY),pain and perceived social constraint were the independent factors(all P<0.05),jointly explaining the total variance by 42.50%.Conclusions The psychological distress of intermediate and advanced lung cancer patients is at a moderate level.The distress is closely related to social constraints.It can be improved by alleviating social constraints and pain relief.
