摘  要:目的分析血清β2微球蛋白(β2-microglobulin,β2-MG)、胱抑素C(Cystatin C,CysC)及脑利钠肽前体(Pro-brain natriuretic peptide,pro-BNP)在老年急性脑梗死(Atherosclerotic cerebral infarction,ACI)患者认知功能障碍中的诊断价值。方法选取老年ACI患者230例为研究组,同期健康体检者225例为对照组。采用颅脑磁共振成像(Magnetic resonance imaging,MRI)和核磁共振弥散加权成像(Diffusion-weighted imaging,DWI)对老年ACI患者梗死病灶进行明确,分别检测两组患者β2-MG、CysC和pro-BNP水平,同时采用美国国立卫生研究组脑卒中量表(National institute of health stroke scale,NIHSS)与蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)评价两组受试者神经功能及认知功能,分析认知功能与血清β2-MG、CysC和pro-BNP的相关性。结果研究组NIHSS评分高于对照组,MoCA评分低于对照组(P<0.05);研究组血清β2-MG、CysC和pro-BNP水平均高于对照组(P<0.05)。血清β2-MG、CysC和pro-BNP水平随着ACI严重程度及神经损伤严重程度呈升高趋势(P<0.05)。认知功能障碍患者血清β2-MG、CysC和pro-BNP水平高于认知功能正常者(P<0.05)。血清β2-MG、CysC和pro-BNP水平与MoCA评分呈负相关,与NIHSS评分呈正相关。结论高β2-MG、CysC和pro-BNP水平可能是ACI发生认知功能障碍的关键危险因素。Objective To investigate the correlation of plasmaβ2-microglobulin(β2-MG),Cystatin C(CysC)and pro-brain natriuretic peptide(pro-BNP)in elderly patients with acute cerebral infarction.Methods 230 elderly patients with acute cerebral infarction were selected as the study group,and 225 healthy people with similar age and gender were selected as the control group.The cerebral magnetic resonance imaging(MRI)and magnetic resonance diffusion weighted imaging(DWI)were used to determine the infarct lesions.The levels of plasmaβ2-MG,CysC and pro BNP in the two groups were detected.At the same time,the national health research group stroke scale was used(NIHSS)and Montreal Cognitive Assessment Scale(MoCA)to evaluate neurological and cognitive function,and the correlation between cognitive function and plasmaβ2-MG,CysC and pro BNP was analyzed.Results The scores of NIHSS were higher in study group than that of control group;MOCA scores were lower than that of control group(P<0.05);the levels ofβ2-MG,CysC and pro-BNP were higher in study group than those of reference group(P<0.05).The levels ofβ2-MG,CysC and pro-BNP were increased in the severity of acute cerebral infarction and nerve injury(P<0.05).The levels ofβ2-MG,CysC and pro-BNP in patients with cognitive impairment were higher than those in patients with normal cognitive function(P<0.05).The levels ofβ2-MG,CysC and pro-BNP were negatively correlated with MoCA score,and positively correlated with NIHSS score.High levels ofβ2-MG,CysC and pro-BNP were important risk factors of cognitive dysfunction in patients with acute cerebral infarction.Conclusion High levels ofβ2-MG,CysC and pro-BNP may be important risk factors for cognitive dysfunction in patients with acute cerebral infarction.
