海南省2型糖尿病患者生命质量评价与分析

摘　　要：目的探讨海南省2型糖尿病患者生命质量及其影响因素,为有效提高糖尿病患者生命质量和制定防治策略提供参考依据。方法本研究采用多阶段分层随机抽样,均衡地理、经济等情况选取海南省4个市县的糖尿病患者作为研究对象,使用二元logistic回归分析EQ-5D-5L量表中不同维度出现问题的影响因素,用Tobit回归分析海南省2型糖尿病患者生命质量的影响因素。结果EQ-5D-5L中健康效用值中位数为0.907(0.782,1)。生命质量的五个维度中“疼痛或不舒服”(52.0%)和“焦虑或沮丧”(46.7%)维度存在问题占比较高。Tobit回归表明,年龄(β=-0.042,P<0.05)、婚姻状态(β=-0.030,P<0.01)、文化程度(β=0.039,P<0.01)、医保类型(β=0.071,P<0.01)、BMI值(β=0.049,P<0.01)、病程(β=-0.003,P<0.05)、疾病危害知晓情况(β=-0.075,P<0.000)、是否患有慢性肾脏疾病(β=0.076,P<0.05)、是否患有心力衰竭(β=0.132,P<0.01)是糖尿病患者生命质量的影响因素。结论海南省糖尿病患者生命质量相对较低,应加强关注女性患者的心理健康状况、开展社区护理干预、提高自身健康素养以及引导患者自我健康管理等措施来改善生命质量。Objective To explore the quality of life and its influencing factors of type 2 diabetes patients in Hainan Province,and to provide a reference for effectively improving the quality of life of diabetic patients and formulating prevention and treatment tactics.Methods In this study,multistage stratified random sampling was used to select patients with type 2 diabetes in 4 cities and counties in Hainan Province as the research object,with balanced geographical and economic conditions.The binary logistic regression was used to analyze the influencing factors of problems in different dimensions of the EQ-5D-5L scale.Tobit regression was used to analyze the influencing factors of the quality of life of patients.Results The median value of health utility in EQ-5D-5L was 0.907(0.782,1).In the five dimensions of life quality,the proportion of issues was relatively high in the dimensions of pain or discomfort(52.0%)and the dimensions of anxiety or depression dimensions(46.7%).Tobit regression showed that age(β=-0.042,P<0.05),marital status(β=-0.030,P<0.01),educational level(β=0.039,P<0.01),type of medical insurance(β=0.071,P<0.01),BMI(β=0.049,P<0.01),course of disease(β=-0.003,P<0.05),awareness of disease hazards(β=-0.075,P<0.000),chronic kidney disease(β=0.076,P<0.05),heart failure(β=0.132,P<0.01)were influencing factors of quality of life.Conclusion The quality of life of patients with type 2 diabetes in Hainan Province is relatively low.We should pay more attention to the mental health status of female patients,carry out community nursing interventions,improve their health literacy,and guide patients to self-health management to improve their quality of life.
