摘  要:目的对照分析新生儿重症监护病房(NICU)双卵双胎和单卵双胎新生儿听力初筛结果,探讨其听力特点及相关因素。方法随机抽取2018年8月17日至2019年7月21日唐山市妇幼保健院NICU的120对双胞胎,共240人,根据受孕方式(体外受精胚胎植入、自然受孕)、孕检B超结果及新生儿的性别将其分为双卵双胎组和单卵双胎组,待生命体征平稳后,采用瞬态耳声发射(TEOAE)仪进行听力筛查,用SPSS 23.0软件统计初筛未通过率,探讨其听力特点及相关因素。结果240人中双卵双胎124人,未通过率37.10%(46/124);单卵双胎116人,未通过率31.90%(37/116)。两组的受孕方式及生产方式相比,差异有统计学意义(P<0.001,P=0.006),其余指标差异无统计学意义。结论双卵和单卵来源对双胎的听力初筛未通过率差异无统计学意义;双卵双胎多为体外受精胚胎植入,单卵双胎多为自然受孕;双卵双胎的剖宫产率高;新生儿的性别及出生体质量<1500 g等因素对听力初筛结果无显著影响。Objective In this study we analyzed the hearing screening results of dizygotic and monozygotic twins in neonatal intensive care unit(NICU),to compare and discuss the hearing characteristics and factors related to positive screening.Methods Overall,120 pairs of twins,totaling 240 infants in the NICU of the Tangshan Maternal and Child Health Hospital were analyzed.Participants were divided into the dizygotic and monozygotic twin groups,and data regarding the method of conception(in vitro fertilization embryo implantation or natural conception),gender,and b-ultrasound results of pregnancy examination were collected.A transient otoacoustic emission(TEOAE)instrument was used for hearing screening after the infants'condition was stabilized,and vital signs had become stable.SPSS 23.0 software was used to calculate the failure rate of initial screening,and the risk factors and related factors were analyzed and summarized.Results In total,we analyzed 240 infants,including 124 dizygotic twins with a failure rate of 37.10%(46/124)and 116 monozygotic twins with a failure rate of 31.90%(37/116).The two groups were significantly different in terms of the methods of conception and delivery(P<0.001,P=0.006),but no significant difference was found in any other indices.Conclusion Dizygotic and monozygotic sources had no significant effect on the failure rate of hearing screening in twins.Dizygotic twins were found to be mostly conceived by in vitro fertilization embryo implantation,while monozygotic twins were predominantly naturally conceived.The cesarean section rate of dizygotic twins was high.Factors such as sex pair and birth weight<1500 g had no significant effect on the results of preliminary screening.
