男性亚急性皮肤型红斑狼疮1例  

Subacute Cutaneous Lupus Erythematosus in a Male Patient: A Case Report

摘　　要：患者男,49岁,反复面部、前胸、四肢红斑8年。皮肤科情况:面部、前胸、双足底对称性红色环状斑片;双臂上段伸侧、双侧肩部褐色环状色素沉着斑;面部、胸前V形区毛细血管扩张;雷诺氏征阳性。实验室检查:ANA阳性,滴度1∶100(胞浆颗粒型);抗SS-A52抗体强阳性。左下颌皮损组织病理示:表皮网篮状角化过度,角化不全,角栓形成,基底细胞空泡变性,真皮浅、中层血管及附属器周围可见带状浸润的淋巴细胞。诊断:亚急性皮肤型红斑狼疮。治疗:口服硫酸羟氯喹,外用0.01%吡美莫司乳膏。患者目前仍在随访中。A 49-year-old male presented with recurrent erythema on face,forehead,and extremities for more than 3 years.Dermatological examination revealed that symmetrical annular erythema on the face,forehead and soles of both feet,annular hyperpigmented spots on the upper part of both arms and bilateral shoulders,capillary dilation on the face and chest,positive Raynaud's sign.Laboratory tests found positive ANA antibody with a titer of 1:100(cytoplasmic granular type),strong positive anti-SS-A52 antibody.Histopathological examination showed epidermal reticular basket hyperkeratosis,vacuolar degeneration of basal cells,lymphocytic infiltration around the superficial and middle dermal vessels and skin appendages.Diagnosis of subacute cutaneous lupus erythematosus(SCLE)was made.Oral hydroxychloroquine sulfate and topical 0.01%pimecrolimus cream were given and the patient was followed up.
