摘  要:目的 :利用磁共振神经成像技术探讨正常成人骶神经影像解剖特征,为骶神经调控手术治疗下尿路功能障碍性疾病提供参考。方法 :收集本院在3.0T MRI机器进行增强3D-STIR-SPACE序列扫描骶神经成像检查的成年患者,选取骶神经形态显示正常者,然后在3D骶神经后处理重建图像上对第1骶神经(S1)~第4骶神经(S4)进行观察,并分别测量每支骶神经连续走行的角度以及在骶神经管内的长度,比较男、女2组间每对及每支骶神经各测量参数差异有无统计学意义。结果 :磁共振骶神经图像中双侧S1、第2骶神经(S2)显示率均为100%,双侧第3骶神经(S3)显示率均为94.6%,双侧S4显示率分别为33.7%、29.4%。每对S1~S4向外走行角度及S1~S3在骶神经管内的长度逐渐减小,S1~S3向前走行的角度逐渐增大,差异有统计学意义。每对骶神经左、右侧走行的角度及在骶神经管内的长度差异均无统计学意义。男性右侧S1、左侧S3向前走行的角度及双侧S1~S3在骶神经管内走行的长度大于女性,差异有统计学意义。S1~S4向外走行的角度,男、女性之间无显著性差异。结论 :骶神经在骶神经管内走行的角度和长度存在性别差异,磁共振神经成像技术可为临床骶神经调控手术的神经定位提供重要参考。Objective:To explore the anatomical characteristics of sacral nerve in normal adults by magnetic resonance neurography so as to provide references for the surgical treatment of lower urinary tract dysfunction disease with sacral neuromodulation.Methods:Adult patients with normal sacral nerve morphology were collected who underwent enhanced 3D-STIR-SPACE sequence scanning of sacral nerves at a 3.0T MRI in our hospital.Then,each pair of sacral nerves from the 1st sacral nerve(S1)to the 4th sacral nerve(S4)were observed on the 3D post-processing reconstruction image of sacral nerves,and the continuous travel angle of each sacral nerve and the length in the sacral neural tube were measured respectively.The differences of measurement parameters of each pair and each sacral nerve between the male and female groups were compared.Results:The display rates of bilateral S1 and the 2nd sacral nerve(S2)nerves were 100%,the display rates of bilateral 3rd sacral nerve(S3)nerves were 94.6%,and the display rates of bilateral S4 were 33.7%and 29.4%,respectively.The outward angle of each pair of S1~S4 and the length of S1~S3 in the sacral neural tube gradually decreased,while the forward angle of S1~S3 gradually increased,which were statistically significant.There were no significant differences in the angle and the length in the sacral neural tube between the left and right sides of each pair of sacral nerves.The forward angle of right S1 and left S3 and the length of bilateral S1~S3 in the sacral neural tube in males were greater than those in females,but there was no significant difference in the outward angle of S1~S4 between the males and the females.Conclusions:There are gender differences in the angle and length of sacral nerve in the sacral neural tube.Magnetic resonance neurography can provide an important reference for the localization of sacral nerve in clinical sacral neuromodulation surgery.
