摘　　要：目的分析腹腔镜下囊肿去顶术与卵巢囊肿切除术对子宫内膜异位症患者围术期体征、盆腔疼痛及卵巢体积的影响。方法选取50例子宫内膜异位症患者,采用随机盲选法分为对照组与治疗组,每组25例。对照组给予腹腔镜下卵巢囊肿切除术治疗,治疗组给予腹腔镜下囊肿去顶术治疗。对比两组患者治疗后围术期指标、盆腔疼痛及卵巢体积。结果治疗组患者手术时间短于对照组、术中出血量少于对照组(P<0.05);两组患者引流管放置人数及住院时间比较,差异无统计学意义(P>0.05)。手术后2周、手术后1个月及3个月,治疗组患者VAS评分均明显低于对照组(P<0.05);两组患者治疗前组间视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05)。治疗组患者术后的卵巢体积(8.42±1.15)cm3大于对照组的(4.92±1.24)cm3(P<0.05)。结论腹腔镜下囊肿去顶术应用于子宫内膜异位症患者治疗中的综合应用价值更高,建议该手术治疗方式在临床中推广使用。Objective To analyze the effects of laparoscopic cyst decapitation and ovarian cyst excision on perioperative signs,pelvic pain and ovarian volume in patients with endometriosis.Methods Fifty cases ofendometriosis patients were randomly divided into control group and treatment group,with 25 cases in each group.The control group was treated with laparoscopic ovarian cystectomy,and the treatment group was treated with laparoscopic cystectomy.Perioperative indicators,pelvic pain and ovarian volume were compared between the two groups.Results The operation time of treatment group was shorter than that of control group,and the intraoperative bleeding was less than that of control group(P<0.05).There was no significant difierence in the number ofpatients with drainage tube placement and length ofstay between the two groups(P>0.05).After 2 weeks,1 month and 3 months,VAS scores in treatment group were significantly lower than those in control group(P<0.05).There was no significant difference in VAS scores between the two groups before treatment(P>0.05).The postoperative ovarian volume in the treatment group(8.42±1.15)cm3 was higher than that in the control group(4.92±1.24)cm3(P<0.05).Conclusion Laparoscopic cyst paring is of higher value in the treatment of patients with endometriosis.It is suggested that this surgical treatment should be popularized in clinic.
