促育汤联合补佳乐治疗FM/TE高龄肾虚血瘀型不孕症患者的妊娠率及对子宫、卵巢动脉血流的影响

摘　　要：目的探讨促育汤联合戊酸雌二醇片(补佳乐)治疗卵泡发育不良(FM)/薄型子宫内膜(TE)高龄肾虚血瘀型不孕症的应用效果。方法选取79例FM/TE高龄肾虚血瘀型不孕症患者,依据治疗方案不同分为A组(24例)、B组(25例)及C组(30例)。A组接受补乐佳治疗,B组接受促育汤治疗,C组接受促育汤联合补乐佳治疗。对比三组中医证候积分、子宫和卵巢动脉血流水平、妊娠率。结果治疗后,三组月经不调、腰膝酸软、倦怠乏力和畏寒肢冷评分均出现不同程度下降,A组与B组评分接近(P>0.05),C组评分下降程度更低(P<0.05)。治疗后,三组子宫卵巢动脉收缩期最大血流速度(PSV)水平均出现不同程度上升,血流搏动指数(PI)、阻力指数(RI)水平均出现不同程度下降,A组与B组接近(P>0.05),C组明显优于A、B组(P<0.05)。C组血人绒毛膜促性腺激素(β-HCG)检验、B超检验妊娠率分别为96.67%、86.67%,均明显高于A组的62.50%、54.17%和B组的64.00%、56.00%(P<0.05)。结论促育汤联合补佳乐治疗FM/TE高龄肾虚血瘀型不孕症可有效改善患者的临床症状,改善子宫、卵巢的血流动力水平,提高妊娠率。Objective To investigate the application effect of Promoting Yu Tang combined with Estradiol valerate tablets(Bujiale)in the treatment of ovarian dysplasia(FM)/thin endometrial(TE)infertility of senile patients with kidney deficiency and blood stasis.Methods 79 FM/TE senile infertility patients with kidney deficiency and blood stasis were divided into group A(24 cases),group B(25 cases)and group C(30 cases)according to different treatment plans.Group A received Bulejia treatment,group B received pro-yu decoction treatment,group C received pro-yu decoction combined with Bulejia treatment.TCM syndrome scores,uterine and ovarian arterial blood flow levels and pregnancy rate were compared among the three groups.Results After treatment,the scores of irregular menstruation,soreness and weakness of waist and knee,fatigue and cold limbs of the three groups were decreased to varying degrees,the scores of group A and group B were close(P>0.05),and the score of group C was lower(P<0.05).Uterine ovarian artery systole level of the three groups increased to varying degrees,and the blood flow pulsation index(PI)and resistance index(RI)levels decreased to varying degrees,group A and group B were close(P>0.05),group C was significantly better than groups A and B(P<0.05).The pregnancy rates of human chorionic gonadotropin(p-HCG)test and B-ultrasound test in group C were 96.67%and 86.67%,respectively,which were sigrdficantly higher than 62.50%and 54.17%in group A and 64.00%and 56.00%in group B(P<0.05).Conclusion Zhenyu Decoction combined with Bujialle in the treatment of FM/TE senile infertility with kidney deficiency and blood stasis can effectively improve the clinical symptoms of patients,improve the blood dynamic level of uterus and ovary,and increase the pregnancy rate.
