超声心动图定量评价肺动脉高压患者右心室功能

摘　　要：目的:探讨超声心动图定量评价肺动脉高压(PAH)患者右心室功能的临床价值。方法:选取2020年1月-2021年1月南京市高淳人民医院收治的49例PAH患者为实验组,依据PAH功能分级标准将其分为两个亚组,实验1组和实验2组,同期49例肺动脉压正常的志愿者为对照组。予以超声心动图检查,测定右心室收缩功能指标三尖瓣环收缩期位移(TAPSE)、右心室面积变化分数(FAC)、三尖瓣侧壁收缩期峰值速度(S'),比较观察指标差异,分析其与肺动脉高压的相关性。结果:实验组TAPSE、FAC、S'平均水平均低于对照组,差异有统计学意义(t=3.730、4.276、3.459,P<0.05)。实验2组TAPSE、FAC、S'阳性率均高于实验1组,TAPSE、FAC、S'平均水平均低于实验1组,差异有统计学意义(t=10.726、11.354、10.420,P<0.05)。随着PAH症状的出现及加重,对照组、实验1组、实验2组三组患者TAPSE、FAC与S'指标水平呈递减趋势,经Spearman分析TAPSE、FAC、S'与PAH呈显著正相关。结论:以超声心动图测定右心室收缩功能指标TAPSE、FAC、S',可以较好的反应PAH患者右室收缩功能,轻症PAH患者右心室收缩功能可以正常,以后随着症状加重,肺血管阻力升高,右心室收缩功能下降。Objective:To investigate the clinical value of quantitative evaluation of right ventricular function by echocardiography in patients with pulmonary hypertension(PAH).Methods:49 patients with PAH admitted to the hospital from January 2020 to January 2021 were selected as the experimental group,and they were divided into experimental group 1 and experimental group 2 according to the functional classification criteria of PAH,and 49 volunteers with normal pulmonary artery pressure in the same period were the control group.Echocardiography was performed,and the right ventricular systolic function indicators tricuspid annular systolic displacement(TAPSE),right ventricular area change fraction(FAC),and tricuspid lateral wall peak systolic velocity(S')were measured and the differences in the observed indicators were compared and their correlation with pulmonary hypertension was analyzed.Results:The mean levels of TAPSE,FAC,and S'in the experimental group were lower than those in the control group,and the differences were statistically significant(t=3.730,4.276,3.459,P<0.05).The positive rates of TAPSE,FAC,and S'in Experiment 2 group were higher than those in Experiment 1 group,and the mean levels of TAPSE,FAC,and S'were lower than those in Experiment 1 group,and the differences were statistically significant(t=10.726,11.354,10.420,P<0.05).As the symptoms of PAH appeared and worsened,the levels of TAPSE,FAC and S'indexes in the three groups of control,experimental 1 and experimental 2 showed a decreasing trend.TAPSE,FAC,and S'were significantly and positively correlated with PAH by Spearman’s analysis.Conclusion:Measurement of right ventricular systolic function indexes TAPSE,FAC,and triple S'by echocardiography can better reflect right ventricular systolic function in patients with PAH.Right ventricular systolic function can be normal in patients with mild PAH,and later decreases as symptoms worsen and pulmonary vascular resistance rises.
