摘 要:目的:分析人工关节置换术结合康复治疗对创伤致掌指(MP)、近侧指间(PIP)关节僵硬患者的效果,为相关研究提供参考。方法:选取2018年7月-2021年1月灵宝市第二人民医院外二科收治的82例MP、PIP关节僵硬患者作为研究对象,根据随机单双数法分为对照组(41例)和观察组(41例)。对照组行人工关节置换术,观察组行人工关节置换术结合康复治疗,对比两种治疗方式下患者关节功能、活动度和疼痛状况。结果:治疗后,观察组患者的上肢功能(Car⁃roll)评分和手部活动度(TAM)评分均高于对照组,差异有统计学意义(t=3.589、2.964,P<0.05);治疗后观察组患者生活质量评分显著高于对照组,差异有统计学意义(t=3.215,P<0.05);护理后观察组患者疼痛评分显著低于对照组,差异有统计学意义(t=2.795,P<0.05);观察组患者并发症发生率显著低于对照组,差异有统计学意义(χ^(2)=4.957,P<0.05)。结论:对MP、PIP关节僵硬患者行人工关节置换术结合康复治疗,可有效提升患者关节功能、活动度,降低疼痛评分和并发症发生率,提升其生活质量。Objective:To analyze the effect of artificial joint replacement combined with rehabilitation therapy on patients with trauma-induced metacarpophalangeal(MP)and proximal interphalangeal(PIP)joint stiffness,and to provide a reference for related studies.Methods:82 patients with MP and PIP joint stiffness admitted to the hospital from July 2018 to January 2021 were selected as study subjects and divided into control(41 cases)and observation groups(41 cases)according to the randomized single-even number method.The control group underwent artificial joint replacement and the observation group underwent artificial joint replacement combined with rehabilitation treatment.The patients’joint function,mobility and pain conditions were compared between the two treatment modalities.Results:After treatment,patients in the observation group had higher upper limb function(Carroll)scores and hand mobility(TAM)scores than the control group,with statistically significant differences(t=3.589,2.964,P<0.05).The quality of life scores of patients in the observation group were significantly higher than those in the control group after treatment,and the difference was statistically significant(t=3.215,P<0.05).The pain scores of patients in the observation group were significantly lower than those in the control group after care,and the difference was statistically significant(t=2.795,P<0.05).The incidence of complications was significantly lower in the observation group than in the control group,and the difference was statistically significant(χ^(2)=4.957,P<0.05).Conclusion:Artificial joint replacement combined with rehabilitation therapy for patients with MP and PIP joint stiffness can effectively improve joint function and mobility,reduce pain scores and complication rates,and improve quality of life.
