基于虚-瘀-变的病机特点治疗臁疮的临床体会

摘　　要：目的:探讨更加经济有效地治疗臁疮的临床方法。方法:回顾性分析2020年12月-2022年10月昆都仑区卜尔汉图卫生院门诊治疗13例臁疮患者的临床资料,男性10例,女性3例;年龄45~72岁,平均年龄59.27岁;均为单侧肢体共计疮面16个,发为内臁11例,外臁2例。基于臁疮“虚-瘀-变”的病机特点,13例臁疮患者均给予补阳还五汤加减口服汤剂治疗,同时进行必要的伤口处理和外敷生肌玉红膏。结果:13例臁疮患者均在短时间内症状明显改善,疮口愈合快,痊愈后不留瘢痕。通过回访,至结稿之日,13例患者均未复发。结论:臁疮具有“虚-瘀-变”的病机特点,“虚-瘀”是臁疮发病的根本病机,“湿、热、毒”是其在“虚-瘀”基础上发生的变化。基于“虚-瘀”为本的病机理论指导,采用益气活血的补阳还五汤为主方,结合生肌玉红膏外用治疗,临床疗效确切。Objective:To explore a more economical and effective clinical method for the treatment of Venous leg ulcer(VLU).Methods:The data of 13 patients with VLU treated in outpatient of Burhantu Health Centre in Kundurun district from December 2020 to October 2022 were analyzed retrospectively.All of them were diagnosed with unilateral limb disease,with a total of 16 ulcers,including 11 cases of internal ulcer and 2 cases of external ulcer.There were 10 males and 3 females,aged 45 to 72,with an average age of 59.27 years.Based on the pathogenetic characteristics of"Deficiency-Blood Stasis-Change",the 13 VUL patients were treated with Buyang Huanwu Decoction plus or minus oral decoction,adding necessary wound treatment and external application of Shengji Yuhong ointment.Results:The symptoms of the 13 VLU patients were significantly improved in a short time,the sores healed quickly,and no scars left after healing.No relapse in these 13 patients were found through the follow-up until the date of this paper being finished.Conclusion:VLU has the pathogenetic characteristics of"Deficiency-Blood stasis-Change"."Deficiency-Blood stasis"is the basic pathogenesis of VLU,and Dampness to Heat to Toxin is the change based on"Deficiency-Blood stasis".Based on the pathogenetic characteristics of"Deficiency-Blood Stasis-Change",the clinical effect is visibly in treating VLU with Buyang Huanwu Decoction to supplement qi and activate blood,combined with the external use of Shengji Yuhong Ointment.
