口腔颌面外科住院患者多重耐药菌感染的临床分析

摘　　要：目的 对某口腔专科医院口腔颌面外科住院患者多重耐药菌(MDRO)感染状况进行调查分析,为有效预防和控制MDRO的医院感染提供依据。方法 采用回顾性研究方法,对2017—2021年口腔颌面外科住院患者进行医院感染监测。结果 调查合格送检细菌培养标本1 672份,分离出目标病原菌1 055株,其中MDRO 142株,检出率13.5%;MDRO以大肠埃希菌、奇异变形杆菌、耐甲氧西林金黄色葡萄球菌、肺炎克雷伯菌和产酸克雷伯氏菌最为多见,其中,产ESBLs大肠埃希菌检出率最高,占45.7%;MDRO主要来源于伤口及下呼吸道,分别占79.3%和18.6%;患者所患疾病类型主要为口腔癌、口腔颌面部感染、口腔良性肿瘤及颌面部创伤,分别占49.3%、24.3%、15.0%和10.0%。结论 某口腔专科医院口腔颌面外科住院患者MDRO感染分布广泛,应加强监测,采取有效措施,预防和控制MDRO的医院感染。Objective To investigate and analyze the infection status of multidrug-resistant organisms(MDRO)in inpatients in department of oral and maxillofacial surgery of a stomatological hospital,so as to provide evidence for effective prevention and control of nosocomial infection caused by MDRO.Methods A retrospective study method was used to monitor nosocomial infection among inpatients in department of oral and maxillofacial surgery from 2017 to 2021.Results A total of 1672 qualified bacterial culture specimens were investigated,and 1055 strains of target pathogens were isolated,including 142 strains of MDRO,with a detection rate of 13.5%.The most common MDRO were Escherichia coli,Proteus mirabilis,methicillin-resistant Staphylococcus aureus,Klebsiella pneumoniae,and Klebsiella oxytoca.Among them,ESBLs-producing Escherichia coli had the highest detection rate,accounting for 45.7%;MDRO were mainly detected from wounds and lower respiratory tract,accounting for 79.3%and 18.6%respectively;the main disease types of patients were oral cancer,oral and maxillofacial infection,oral benign tumor and maxillofacial trauma,accounting for 49.3%,24.3%,15.0%and 10.0%respectively.Conclusion The MDRO infection is widely distributed among inpatients in department of oral and maxillofacial surgery of a stomatology hospital.Monitoring should be strengthened and effective measures should be taken to prevent and control nosocomial infection of MDRO.
