摘 要:目的 探讨三维经阴式彩超检查用于临床鉴别诊断子宫畸形疾病的准确性与应用价值。方法 选择2021年1月-2022年6月江阴市人民医院收诊的疑似子宫畸形患者42例作为研究对象,分别进行二维经阴式彩超检查与三维经阴式彩超检查,以宫腔镜检查结果作为金标准,归纳总结42例疑似子宫畸形患者诊断结果,同时计算二维彩超与三维彩超检查灵敏度、特异度与准确率,比较统计学差异性。归纳总结不同类型子宫畸形患者影像学表现。结果 42例疑似子宫畸形患者通过宫腔镜检查结果显示,31例患者确诊为子宫畸形,患病率为73.81%。其中7例患者诊断为纵隔子宫,5例患者诊断为双角子宫,5例患者诊断为单角子宫,4例患者诊断为双子宫,9例患者诊断为弓状子宫,1例患者诊断为幼稚子宫。通过计算,二维彩超检查的灵敏度为61.29%、特异度为45.45%、准确率为57.14%;三维经阴式彩超检查的灵敏度为96.77%、特异度为90.91%、准确率为95.24%,三维彩超灵敏度、准确率均明显高于二维彩超(P<0.05)。结论 临床鉴别诊断子宫畸形疾病采用三维经阴式彩超检查技术,可清晰观察患者子宫情况,该检查技术具有极高的灵敏度与准确率,能够提供影像支持,值得推广。Objective To explore accuracy and application value of three-dimensional transvaginal color Doppler ultrasound in clinical differential diagnosis of uterine malformation diseases.Methods The paper chose 42 patients with suspected uterine malformations admitted to Jiangyin People's Hospital from January 2021 to June 2022 as research objects,treated with dimensional transvaginal ultrasound and three dimensional transvaginal ultrasound respectively.With gold standard of hysteroscopy results,diagnostic result of 42 patients with suspected uterine malformations was summarized.At the same time,sensitivity,specificity,and accuracy of two-dimensional and three-dimensional ultrasound was calculated,and statistical difference was compared.Imaging manifestations of patients with different types of uterine malformations were summarized.Results Hysteroscopy examination showed,31 of 42 suspected patients with uterine malformations were diagnosed with uterine malformations,with prevalence rate of 73.81%.Among them,7 patients were diagnosed with mediastinal uterus,5 with bicornuate uterus,5 with monocornuate uterus,4 with bicornuate uterus,9 with arcuate uterus,and 1 with immature uterus.Through calculation,sensitivity of two-dimensional ultrasound examination was 61.29%,specificity was 45.45%,accuracy was 57.14%.Sensitivity,specificity,and accuracy of three-dimensional transvaginal ultrasound examination were 96.77%,90.91%,and 95.24%respectively.Sensitivity and accuracy of three-dimensional ultrasound was significantly higher than two-dimensional ultrasound(P<0.05).Conclusion During clinical differential diagnosis,three-dimensional transvaginal color ultrasound examination technology can clearly observe uterus condition of uterine malformation disease,can provide imaging support with high sensitivity and accuracy,which is worthy of promotion.
