摘　　要：目的 探讨新生儿呼吸道合胞病毒急性下呼吸道感染(RSV-ALRTI)的规范治疗方案。方法 选取2017年11月-2018年3月山西省儿童医院新生儿内科收治的59例RSV-ALRTI新生儿为基线组,行低级抗菌素治疗,另选2018年11月-2019年3月收治的62例RSV-ALRTI新生儿为目标组,根据抗菌素不合理使用情况组建多学科团队,进行多学科督导管理,比较治疗情况。结果 目标组平均DoT比基线组明显下降;目标组主要症状持续时间、肺部体征消失时间、住院时间、呼吸机使用和吸氧时间较基线组短(P<0.05);目标组总DoT/总住院时间比较基线组低(P<0.05),目标组平均住院费用较基线组少(P<0.05);目标组并发症发生率较基线组低(P<0.05)。结论 经多学科团队管理后,RSV-ALRTI新生儿诊疗中的抗菌素不合理使用率明显下降,医疗费用、并发症明显减少。Objective To explore the standard treatment plan for neonatal respiratory syncytial virus acute lower respiratory tract infection(RSV-ALRTI).Methods From November 2017 to March 2018,59 neonates with RSV-ALRTI who were admitted to the Department of Neonatal Medicine of Shanxi Children's Hospital were selected as the baseline group and received low-grade antibiotic therapy.Another 62 neonates with RSV-ALRTI who were treated from November 2018 to March 2019 were selected as the target group,and a multidisciplinary team was formed according to the unreasonable use of antibiotics to conduct multidisciplinary supervision and management,to compare treatments.Results The mean DoT of the target group was significantly lower than that of the baseline group;the duration of main symptoms,disappearance time of pulmonary signs,hospitalization time,ventilator use and oxygen inhalation time in the target group were shorter than those in the baseline group(P<0.05);the total DoT/total length of hospital stay in the target group was lower than that in the baseline group(P<0.05);the average hospitalization cost of the target group was less than that of the baseline group(P<0.05);the incidence of complications in the target group was lower than that in the baseline group(P<0.05).Conclusion After multidisciplinary team management,the rate of irrational use of antibiotics in the diagnosis and treatment of neonates with RSV-ALRTI was significantly reduced,and the medical expenses and complications were significantly reduced.
