养血祛风生发汤联合复方甘草酸苷胶囊治疗斑秃临床研究

摘　　要：目的:观察自拟养血祛风生发汤联合复方甘草酸苷胶囊治疗斑秃的临床疗效及其对血清炎症因子的影响。方法:将108例斑秃患者随机分为治疗组与对照组各54例。对照组采用复方甘草酸苷胶囊治疗,治疗组在对照组的基础上联合养血祛风生发汤治疗。观察比较2组临床疗效、生活质量评分、不良反应发生率,以及外周血Th17细胞、Treg细胞、血清白细胞介素-6 (IL-6)、白细胞介素-23 (IL-23)水平变化。结果:治疗组总有效率为96.30%,高于对照组79.63%,组间比较,差异有统计学意义(P<0.05)。治疗前,2组生理功能、心理状况、社会功能等生活质量评分比较,差异无统计学意义(P>0.05);治疗后,2组上述各项生活质量评分均较治疗前升高(P<0.05),且治疗组各项评分均高于对照组(P<0.05)。治疗前,2组外周血Th17细胞、Treg细胞水平比较,差异无统计学意义(P>0.05);治疗后,2组Th17细胞水平较治疗前降低(P<0.05),Treg细胞水平较治疗前升高(P<0.05);且治疗组2项指标改善较对照组更显著(P<0.05)。治疗前,2组血清IL-6、IL-23水平比较,差异无统计学意义(P>0.05);治疗后,2组血清IL-6、IL-23水平均较治疗前降低(P<0.05),且治疗组2项指标均低于对照组(P<0.05)。治疗期间,治疗组不良反应发生率为5.56%,对照组为7.41%,组间比较,差异无统计学意义(P>0.05)。结论:养血祛风生发汤联合复方甘草酸苷胶囊治疗斑秃可提高临床疗效,有效调节外周血Th17、Treg细胞水平,降低IL-6、IL-23水平及不良反应发生率,提高患者生活质量。Objective:To observe the clinical effect of Yangxue Qufeng Shengfa Decoction combined with Compound Glycyrrhizin Capsules for alopecia areata and its effect on inflammatory factors in serum.Methods:A total of 108 cases of patients with alopecia areata were randomly divided into the treatment group and the control group,with 54 cases in each group.The control group was treated with Compound Glycyrrhizin Capsules,and the treatment group was additionally treated with Yangxue Qufeng Shengfa Decoction based on the treatment of the control group.The changes in levels of Th17 cells and Treg cells in peripheral blood and interleukin-6(IL-6)and interleukin-23(IL-23)in serum,as well as the clinical effects,scores of quality of life,incidence of adverse reactions were observed and compared between the two groups.Results:The total effective rate was 96.30%in the treatment group,higher than that of 79.63%in the control group,the difference being significant between the two groups(P<0.05).Before treatment,there was no significant difference being found in the comparison of the scores of quality of lifeincluding physical functioning,psychological condition and social role functioning between the two groups(P>0.05);after treatment, the above scores of quality of life in the two groups were increased whencompared with those before treatment (P<0.05),and the scores in the treatment group were higher thanthose in the control group (P<0.05). Before treatment,there was no significant difference being found inthe comparison of the levels of Th17 cells and Treg cells in peripheral blood between the two groups (P>0.05);after treatment, the levels of Th17 cells in the two groups were decreased when compared withthose before treatment (P<0.05), and the levels of Treg cells in the two groups were increased whencompared with those before treatment (P<0.05);the improvement of the two indexes in the treatmentgroup was more significant than that in the control group (P<0.05).
