摘 要:目的 研究多层螺旋CT、磁共振成像诊断胸腰椎体结核感染的价值。方法 选择2019年6月至2022年6月河南省洛阳正骨医院收治的78例胸腰椎体结核感染患者进行观察,统计并整理所有患者的病历资料,分别采取多层螺旋CT、磁共振检查,统计不同检查的检查时间及等待结果时长,并比较其病灶检出率及成像评分,分析不同检查诊断疾病的价值。结果 磁共振的检查时间短于多层螺旋CT,但等待检查结果时长大于多层螺旋CT,差异有统计学意义(P <0.05)。多层螺旋CT中死骨形成的检出率高出磁共振,但椎旁脓肿、椎体破坏、椎间盘受累的检出率均低于磁共振,差异有统计学意义(P <0.05)。多层螺旋CT中死骨形成的成像评分高出磁共振,但椎旁脓肿、椎体破坏、椎间盘受累的成像评分均低于磁共振,差异有统计学意义(P <0.05)。结论 磁共振在疾病中的检出率较高,同时成像质量较高,可成为胸腰椎体结核感染诊断的主要方式。Objective:To study the clinical value of multi-slice spiral CT and magnetic resonance imaging on the diagnosis of thoracolumbar vertebral tuberculosis.Methods A total of 78 thoracolumbar vertebral tuberculosis patients admitted from June 2019 to June 2022 in Henan Luoyang Orthopedic Hospital were selected.Medical records of all patients were counted and compiled.The multi-slice spiral CT and magnetic resonance imaging were performed,respectively.The examination time and the time of waiting results for different tests were counted,the detection rate and imaging score were compared,and the value of different tests for diagnose diseases were analyzed.Results The examination time based on the magnetic resonance imaging was shorter than that based on the multi-slice spiral CT;but the time of waiting results was longer than that based on the multi-slice spiral CT,with significant difference(P<0.05);The detection rate of sequestration based on the multi-slice spiral CT was higher than that based on the magnetic resonance imaging,the detection rates of vertebral abscess,vertebral damage and intervertebral disc involvement were lower than those based on the magnetic resonance imaging,with significant differences(P<0.05);The imaging score of sequestration based on the multi-slice spiral CT was higher than that based on the magnetic resonance imaging,the imaging scores of vertebral abscess,vertebral damage and intervertebral disc involvement were lower than those based on the magnetic resonance imaging,showing statistically significant differences(P<0.05).Conclusion The magnetic resonance imaging has the higher disease detection rates and imaging quality for thoracolumbar vertebral tuberculosis.It can become the main way of diagnosis of thoracolumbar vertebral tuberculosis.
