心可舒防治氟尿嘧啶类药物相关性心脏毒性的临床观察

摘  要:目的 观察心可舒防治氟尿嘧啶类药物相关性心脏毒性的临床疗效。方法 选取2019年1月至2021年12月赣南医学院第一附属医院和赣州市肿瘤医院经含氟尿嘧啶类药物方案(如奥沙利铂+卡培他滨方案,或单药卡培他滨方案)治疗的结直肠肿瘤患者68例,根据患者的治疗意愿性随机均分为观察组和对照组,对照组予以含氟尿嘧啶类药物方案治疗,观察组在对照组的基础上予以心可舒治疗,两组治疗时长为指南推荐的24周或不耐受换用其他治疗方案,比较两组患者耐受疗程数情况,近期临床获益,Karnofsky评分,毒性评价(如心电图异常情况、临床症状等)。结果 治疗后,观察组耐受疗程数的中位值、总有效率及Karnofsky评分均高于对照组(P<0.05)。观察组总的心脏毒性不良反应发生率、心悸与血压异常的临床症状发生率均低于对照组(P<0.05)。结论 心可舒可以改善氟尿嘧啶类药物相关性心脏毒性,增加患者治疗的耐受性,与氟尿嘧啶类药物联合使用,具有较好的增效减毒作用。Objective To investigate the effect of xinkeshu on treatment of cardiac toxicity of fluorouracil.Methods 68 patients with colorectal tumor who were treated with fluorouracil-containing regimens(such as oxaliplatin plus capecitabine,or capecitabine regimen)in the First Affiliated Hospital of Gannan Medical University and Ganzhou Cancer Hospital from January 2019 to December 2021 were selected and randomly divided into observation group(treated with xinkeshu plus fluorouracil-containing regimen)and control group(treated with fluorouracil-containing regimen)according to their treatment willingness.The duration of treatment course of the two groups was up to 24 weeks recommended by the guideline or intolerance was changed to other treatment plan.The number of tolerance courses,short-term clinical efficacy,Karnofsky score and toxicity evaluation(such as electrocardiogram abnormalities and clinical symptoms,etc.)of the two groups were compared.Results After treatment,the median number of treatment courses,total effective rate and Karnofsky score in the observation group were significantly higher than those in the control group(P<0.05).In addition,the adverse reactions incidence of all cardiotoxic,as well as the incidence of clinical symptoms of palpitation and abnormal blood pressure in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Xinkeshu combined with fluorouracil regimens has good synergism that can regulate the cardiac toxicity of fluorouracil,which have positive effects on increasing the tolerance of treatment in patients and reducing side effects.
