基于Meta分析探讨慢性心力衰竭中医证型与B型利钠肽的相关性

摘　　要：目的:系统评价慢性心力衰竭中医证型与B型利钠肽的相关性。方法:检索中国知网、万方、维普、the Cochrane Library、EMbase、PubMed、Web of Science数据库,收集慢性心力衰竭中医证型与B型利钠肽相关性的文献,检索时限为建库至2022年3月6日,采用RevMan 5.4.1软件进行Meta分析。结果:共纳入9篇文献。Meta分析结果显示,慢性心力衰竭阳虚水泛证B型利钠肽与气虚血瘀证[MD=632.88,95%CI(338.92,926.85),P<0.000 1]、气阴两虚证[MD=1 261.04,95%CI(739.06,1 783.01),P<0.000 01]、心肺气虚证[MD=343.87,95%CI(271.08,416.66),P<0.000 01]、心肾阳虚证[MD=388.30,95%CI(101.16,856.22),P<0.000 1]比较,差异有统计学意义。结论:现有证据表明,B型利钠肽水平偏高可作为诊断慢性心力衰竭阳虚水泛证的参考。Objective:To systematically evaluate the correlation between Chinese medical syndrome type of chronic heart failure and B-type natriuretic peptide.Methods:The literatures of the Chinese medical syndrome type of chronic heart failure and B-type natriuretic peptide were collected by CNKI,WanFang,VIP,the Cochrane Library,EMbase and PubMed databases from the establishment of the database to March 6,2022.RevMan 5.4.1 software was used for Meta-analysis.Results:A total of 9 literatures were included.Meta-analysis results showed that there were significant differences in B-type natriuretic peptide between chronic heart failure with Yang deficiency and water overflow syndrome,Qi deficiency and blood stasis syndrome[MD=632.88,95%CI(338.92,926.85),P<0.0001],Qi and Yin deficiency syndrome[MD=1261.04,95%CI(739.06,1783.01),P<0.00001],heart-lung Qi deficiency syndrome[MD=343.87,95%CI(271.08,416.66),P<0.00001],and heart-kidney Yang deficiency syndrome[MD=388.30,95%CI(101.16,856.22),P<0.0001].Conclusion:The current evidence showed that a high level of B-type natriuretic peptide can be used as a reference for the diagnosis of chronic heart failure with Yang deficiency and water overflow syndrome.
